摘要
目的研究成人急性髓系白血病诱导化疗缓解后强化巩固治疗的必要性、安全性,使其生存率提高。方法参加研究的患者为我院201203-201703月经骨髓穿刺等相关检查确诊为急性髓系白血病的成人患者。每组30例,包括对照组和治疗组。对照组仅给予常规化疗,治疗组给予大剂量阿糖胞苷强化巩固化疗。观察2组疾病控制效果及用药后发生的不良反应情况。结果对照组较治疗组5年生存率低,P<0.05。2组骨髓抑制及消化道反应比较不明显,P>0.05。而治疗组中虽然全部患者都出现不良反应,包括因抵抗力降低而发生感染、消化系统症状、甚至造血系统抑制等,但均安全度过,不影响疾病恢复。结论应用大剂量阿糖胞苷对处于缓解期的急性髓系白血病患者进行强化巩固治疗,化疗风险及副作用小,病人容易承受,可以明显提高远期预后。
Objective To study the necessity and safety of consolidation therapy in adult patients with acute myeloid leukemia in?
duced to complete remission to improve disease-free survival. Methods The patients participated in the study were adult patients withacute myeloid leukemia diagnosed through bone marrow aspiration and other related examinations in our hospital from March 2012 to March 2017. There were 30 cases in each group, including the control group and the treatment group. The control group was given con?ventional chemotherapy only, while the treatment group was given high-dose cytarabine consolidation chemotherapy. Observe the con?trol effect of the two groups and the adverse reactions after taking the medicine. Results The 5 years of disease-free survival rate of thecontrol group was lower than that of the treatment group, P 〈 0.05. Comparing the two groups, the myelosuppression and the reaction ofdigestive tract were not obvious, P 〉 0.05. All the patients in the treatment group had adverse reactions, including infection, digestivesymptoms and hematopoietic system suppression due to lower resistance, but they all recovered. Conclusion High-dose cytarabine canenhance the consolidation therapy in patients with acute myeloid leukemia at remission stage. Because of its low risk and light side ef?fects, the patients found easy to bear. Therefore, it can significantly improve long-term prognosis.
