白细胞介素18启动子607C/A和137G/C基因多态性与冠心病相关性的Meta分析 被引量:2

Correlation between polymorphisms of 607C/A and 137G/C, promoters of interleukin-18, and coronary artery disease: a Meta-analysis

摘要 目的探讨白细胞介素18(IL-18)启动子607C/A和137G/C基因多态性和冠心病的相关性。方法计算机检索Pub Med、EMbase、Web of Science、Science Direct、中国知网、维普中文科技期刊数据库、万方数据库,查找并筛选出相关的病例对照研究。同时查阅检索结果中所附相似文献及参考文献。检索时限均为各数据库建库至2017年2月26日。由两名评价员单独进行文献筛选及资料提取,采用Stata SE 12.0软件进行Meta分析以及其他相关统计学分析。结果 IL-18启动子607C/A在亚洲人群中,拥有等位基因A的人群冠心病的发病风险(OR=0.628,95%CI:0.533~0.740,P<0.001)较低,在高加索人群中未发现此相关性(OR=0.950,95%CI:0.803~1.124,P=0.548)。使用显性基因模型进行分析发现IL-18启动子607C/A基因的AA/CA基因型人群与冠心病的发病风险呈负相关(OR=0.431,95%CI:0.326~0.570,P<0.001),在高加索人群中未发现此相关性。另外,IL-18启动子137G/C基因的多态性与亚洲人群(OR=0.843,95%CI:0.577~1.231)和高加索人群(OR=1.071,95%CI:0.807~1.421)的冠心病发病风险均无统计学相关性。结论 IL-18启动子607C/A基因的多态性位点A对亚洲人群冠心病有保护性作用,而137G/C基因多态性与冠心病发病无相关性。本研究结果仍需更多大样本、多中心的研究进一步证实。 Objective To investigate the correlation between polymorphisms of 607 C/A and 137 G/C, promoters of interleukin-18(IL-18), and coronary artery disease(CHD). Methods The databases of Pub Med, EMbase, Web of Science, Science Direct, CNKI, VIP and Wan Fang Data were retrieved with computer for searching and screening relative case-control studies. Meanwhile similar studies and references in retrieved results were consulted. The retrieving duration was from establishment time of each database to Feb. 26, 2017. The studies were screened and data extracted by 2 reviewers independently, and then given a Meta-analysis and other relevant statistical analysis by using Stata SE 12.0 software. Results In Asian population, risk of CHD was lower in population with allele A of IL-18 607 C/A polymorphism(OR=0.628, 95%CI: 0.533-0.740, P0.001), and this correlation was not observed in Caucasian population(OR=0.950, 95%CI: 0.803-1.124, P=0.548). The analysis by dominant gene model showed that the population with AA/CA genotypes of IL-18 607 C/A polymorphism was negatively correlated to risk of CHD(OR=0.431, 95%CI: 0.326-0.570, P0.001), and this correlation was not observed in Caucasian population. The polymorphism of IL-18 137 G/C had no statistical correlation with risk of CHD in Asian population(OR=0.843, 95%CI: 0.577-1.231) and Caucasian population(OR=1.071, 95%CI: 0.807-1.421). Conclusion Allele A of IL-18 607 C/A polymorphism has a preventive effect on Asian population from SHD, and 137 G/C polymorphism is not correlated to CHD risk. More studies with larger sample and multicenter are needed to further verify the conclusion.
