CABG术后患者左心房容积指数与肺动脉嵌顿压的相关性研究

Correlation between left atrial volume index and pulmonary capillary wedge pressure in patients after coronary artery bypass grafting

摘要 目的运用超声心动图技术测量冠状动脉搭桥(CABG)术患者的左心房容积指数和肺动脉嵌顿压(PCWP),以探究两者的相关性。方法心脏搭桥患者术后运用心脏超声机测量胸骨旁切面长轴左心房的前后径(LAD),心尖四腔切面左心房的上下径(SA)和左右径(SB)。利用公式计算左心房容积(LAV),体表面积(BSA)和左心房容积指数(LAVI),研究其相关性。结果经Pearson相关分析发现肺动脉嵌顿压(PCWP0)与扩容前长轴左心房的前后径(LAD0)、LAVI呈正相关,与SA0、SB0无相关性。校正高血压病史、体表面积后,发现LAVI与PCWP0(r=0.446,P=0.033)、LAD0(r=0.740,P<0.001)呈正相关。结论心脏搭桥术患者的左心房容积指数与PCWP正相关。 Objective To study the correlation between left atrial volume index(LAVI) and pulmonary capillary wedge pressure(PCWP) detected by using echocardiogram in patients after coronary artery bypass grafting(CABG). Methods The left atrial anteroposterior diameter(LAD) of parasternal section long axis, and left atrial suproinferior diameter(SA) and mediolateral diameter(SB) of apical 4-cavity section were detected by using echocardiogram in patients after CABG. The left atrial volume(LAV), body surface area(BSA) and left atrial volume index(LAVI) were calculated and correlation between them was studied. Results The results of Pearson correlation analysis showed that PCWP0 was positively correlated to LAD0 and LAVI, and was not correlated to SA0 and SB0. After correcting hypertension history and BSA, LAVI was positively correlated to PCWP0(r=0.446, P=0.033) and LAD0(r=0.740, P〈0.001). Conclusion LAVI is positively correlated to PCWP in patients after CABG.
