丹参川芎嗪联合激素方案治疗肾病综合征43例临床评价

Clinical Evaluation on Salvia Miltiorrhiza Ligustrazine Combined with Hormone in the Treatment of 43 Patients with Nephrotic Syndrome

目的探讨丹参川芎嗪联合激素方案治疗肾病综合征(NS)的疗效及对肾功能和炎症、脂质及凝血指标水平的影响。方法选择医院2016年1月至2017年6月收治的NS患者86例,随机分为对照组与观察组,各43例。对照组患者接受常规激素方案治疗,观察组患者加用丹参川芎嗪注射液治疗。结果观察组总有效率为83.72%,明显高于对照组的65.12%(P<0.05);治疗后,两组患者血肌酐(SCr)、尿素氮(BUN)、24 h尿蛋白定量(24 h Up)、C反应蛋白(CRP)、总胆固醇(TC)、三酰甘油(TG)水平均明显降低(P<0.05),白蛋白(ALB)水平明显上升(P<0.01),观察组血清纤维蛋白原(Fib)及D-二聚体(D-D)水平均明显降低(P<0.05),且观察组SCr,24 h Up,CRP,TC,TG,Fib,D-D水平均明显低于对照组(P<0.05),ALB水平明显高于对照组(P<0.01);两组不良反应发生率相当(9.30%比11.63%,P>0.05)。结论丹参川芎嗪联合糖皮质激素治疗NS疗效肯定,可明显改善患者肾功能,降低Fib,D-D,TC,TG水平,减轻炎性反应程度,且不良反应发生率低,安全性高,值得临床推广。

Objective To investigate the curative effect of salvia miltiorrhiza ligustrazine combined with hormone in the treatment of nephrotic syndrome（NS） and its effect on renal function,the levels of inflammation,lipid and coagulation indexes.Methods Totally 86 patients with NS admitted to our hospital from January 2016 to June 2017 were selected and randomly divided into the control group and the observation group,43 cases in each group.The control group was treated with routine hormone regimen,on this basis,the observation group was additionally treated with Salvia Miltiorrhiza Ligustrazine Injection.Results The total effective rate of the observation group was 83.72%,which was significantly higher than 65.12% of the control group（P〈0.05）.After treatment,the levels of SCr,BUN,24 h Up,CRP,TC and TG in the two groups were significantly decreased,while the levels of ALB were significantly increased,the levels of Fib and D-D in the observation group were significantly decreased（P〈0.05）,and the levels of SCr,24 h Up,CRP,TC,TG,Fib and D-D in the observation group were significantly lower than those in the control group（P〈0.05）,while the level ofALB in the observation group was higher than that in the control group（P〈0.01）.There was no significant difference in the incidence rate of adverse reactions between the two groups（9.30% vs.11.63%,P〉0.05）.Conclusion Salvia miltiorrhiza ligustrazine combined with glucocorticoid is effective in the treatment of NS,it can significantly improve the renal function of the patients,reduce the levels of Fib,D-D,TC and TG,reduce the degree of inflammatory reaction with low incidence of adverse reactions and high safety.It is worthy of clinical promotion.
