徐州地区妊娠期糖尿病孕妇血清25羟维生素D_3水平研究

Study on Levels of Serum 25-Hydroxyvitamin D_3 of Pregnant Women with Gestational Diabetes Mellitus in Xuzhou City

目的对比分析徐州地区妊娠期糖尿病(GDM)与正常孕妇的夏季和冬季血清25羟维生素D_3[25-(OH)D_3]水平的差异,为妊娠期尤其是GDM孕妇是否需要补充维生素D提供依据。方法选择2016年夏季和冬季在徐州市妇幼保健院产科门诊进行产前保健的孕妇200例,分为夏季GDM组、冬季GDM组、夏季对照组和冬季对照组,各50例。采用免疫层析法测定各组孕妇血清25-(OH)D_3水平。结果 4组孕妇的血清25-(OH)D_3水平[夏季GDM组为(49.94±7.94)nmol/L、冬季GDM组为(40.41±13.46)nmol/L,夏季对照组为(55.23±12.06)nmol/L,冬季对照组为(46.17±14.72)nmol/L],均低于正常水平,冬季及夏季GDM组的25-(OH)D_3水平分别显著低于冬季及夏季对照组(P<0.05);冬季GDM组25-(OH)D_3水平显著低于夏季GDM组(P<0.05),冬季对照组25-(OH)D_3水平显著低于夏季对照组(P<0.05)。各组维生素D不足和缺乏的比例高达82%~92%,其中冬季不足和缺乏的比例更高,尤其是冬季GDM组25-(OH)D_3水平缺乏比例最高。结论徐州地区孕妇血清25-(OH)D_3水平普遍不足甚至缺乏,GDM孕妇维生素D缺乏情况比正常妊娠孕妇更为严重,冬季比夏季严重,孕妇尤其是GDM孕妇在冬季更要注意维生素D滴剂的补充。

Objective To compare the difference in levels of serum 25-hydroxyvitamin D3 [ 25-（OH） D3 ] between the pregnant women with gestational diabetes mellitus（GDM） and the normal pregnant women in summer and winter in Xuzhou City,in order to provide evidence for the requirement of vitamin supplement in pregnant women,especially those with GDM. Methods Totally 200 pregnant women received prenatal care in the obstetrical clinic of the Xuzhou Maternity and Child Health Care Hospital in summer and winter of 2016 were selected and divided into the summer GDM group,winter GDM group,summer control group and winter control group,50 cases in each group. The level of serum 25-（OH） D3 was determined in all the pregnant women by using immunochromatographic method. Results The levels of serum 25-（OH） D3 in the 4 groups were lower than the normal level [ summer GDM group：（49. 94 ±7. 94） nmol/L,winter GDM group：（40. 41 ± 13. 46） nmol/L,summer control group：（55. 23 ± 12. 06） nmol/L,winter control group：（46. 17 ± 14. 72） nmol/L ]. The levels of 25-（OH） D3 in the winter GDM group and summer GDM group were significantly lower than those in the winter control group and summer control group,respectively（P〈0. 05）. The level of 25-（OH） D3 in the winter GDM group was significantly lower than that in the summer GDM group（P〈0. 05）. The level of 25-（OH） D3 in the winter control group was significantly lower than that in the summer control group（P〈0. 05）. The percentage of vitamin D inadequacy and deficiency in each group reached up to 82%-92%,with higher percentage of inadequacy and deficiency in winter. The percentage of deficient 25-（OH） D3 level in winter GDM group was the highest. Conclusion The level of 25-（OH） D3 in pregnant women in Xuzhou City was generally inadequate or even deficient. Vitamin D deficiency was severer in pregnant women with GDM than normal pregnant women. In winter,vitamin D deficiency was severer than that in summer. Therefore,pregnant women,especially those with GDM,should pay more attention to vitamin D supplementation.
