无痛分娩联合自由体位对初产妇分娩的影响

Effect of painless labor combined with free body position on primipara delivery

目的:探讨无痛分娩联合自由体位对初产妇分娩的影响。方法:选取100例初产妇为研究对象,按照产妇意愿分为对照组和观察组,各50例。对照组采用常规体位分娩,观察组采用无痛分娩联合自由体位方法分娩,比较两组产妇分娩总产程时间、产后出血量、剖宫产率、新生儿窒息率以及疼痛等级评分。结果:观察组产妇剖宫产率为18.0%(9/50),低于对照组的36.0%(18/50)(χ2=4.009,P=0.040);观察组和对照组的新生儿窒息率均为2.0%(1/50),组间比较,差异无统计学意义(χ2=0.001,P=1.000);观察组产妇总产程时间为(452.54±122.37)min,短于对照组的(602.27±143.58)min(t=5.610,P=0.032);观察组和对照组产妇产后出血量分别为(130.24±31.11)mL和(128.86±32.44)mL,组间比较,差异无统计学意义(t=0.211,P=0.412);观察组产妇疼痛评分等级优于对照组(P<0.05)。结论:无痛分娩联合自由体位在初产妇分娩中应用可降低产妇剖宫产率,减轻产妇疼痛程度,缩短分娩总产程时间,值得推广应用。

Objective： To explore the effect of painless labor combined with free body position on primipara delivery. Methods： 100 cases of primiparas were selected and divided into control group and research group according to their wishes. The control group（50 cases） was given routine position delivery,and the study group（50 cases） was given painless labor combined with free body position. The total labor time,postpartum hemorrhage volume,the rates of cesarean section and neonatal asphyxia and pain grade score were compared between the two groups. Results： The cesarean section rate in the research group（18. 0%,9/50） was lower than that in the control group（36.0 %,18/50）（χ^2= 4. 009,P = 0.040）; the total labor time in the research group（452. 54 ± 122. 37 min） was shorter than that in the control group（602.27 ± 143.58 min）（t = 5. 610,P = 0.032）; the pain grade score of the two groups were statistically significant（P〈0.05）. There was no significant difference in neonatal asphyxia rate（2.0 %,1/50; χ^2= 0.001,P = 1.000） and postpartum hemorrhage（130.24 ± 31.11 mL,128.86 ± 32.44 mL; t= 0.211,P = 0.412） between the two groups. Conclusion： Painless labor combined with free posture in primipara childbirth can reduce the cesarean section rate,relieve the patients＇ pain and shorten the total labor time,which is worth popularizing and applying.
