股骨颈隐性创伤性骨折的CT和MRI的对比

摘要 目的:对比分析CT和MRI对股骨颈隐性创伤性骨折诊断价值,以提高对本病的认识。方法:收集2010年1月至2016年12月共15例经临床及手术证实的股骨颈隐性创伤性骨折的临床和影像学资料进行回顾性分析。结果:15例患者中,男性6例、女性9例;右侧股骨颈骨折9例、左侧6例;在X线平片上15例股骨颈表现为正常或可疑骨折,在CT上4例股骨颈表现为正常或可疑骨折,在MRI上14例股骨颈可见骨皮质连续性中断及骨髓水肿,1例为正常。结论:和CT相比,MRI更能准确的评价股骨颈隐性创伤性骨折。对具有股骨颈骨折临床表现的患者,CT检查阴性时应进一步行MRI检查。 Objective: To analyze comparatively the diagnostic value of CT and MRI on patients with occult femoral neck traumatic fractures so as to improve the recognition of radiology manifestation of the disease. Methods: Clinical data and image findings of 15 cases of patients with occult femoral neck traumatic fractures proved by surgical treatment and clinical course,observed from January 2010 to December 2013,were analyzed retrospectively. Results: In the 15 patients(6 males and 9 females),there were 9 cases with occult femoral neck traumatic fractures in right and 6 cases in left; 15 femoral necks were identified as negative or suspected fractures by x-ray and 4 cases by CT,and 14 cases were identified as interruption of bone cortical continuity and edema of bone marrow and 1 case was negative by MRI. Conclusion: MRI is deemed more precise in the diagnosis of occult femoral neck traumatic fractures in comparison to CT,and further MRI examination should be performed when the CT examination is negative.
