肛周脓肿并肛瘘一次性根治术的效果

Effect of primary radical surgery on perianal abscesses combined with anal fistulas

目的:分析肛周脓肿合并肛瘘进行一次性根治术的治疗效果。方法:150例合并肛瘘的肛周脓肿患者作为观察对象,随机分为两组,每组75例;接受分期引流治疗术的患者为对照组,接受一次性根治术治疗者为观察组,分析两组患者的治疗效果及疾病复发情况。结果:观察组患者术后疼痛时间及住院时间较对照组短(P<0.05),住院费用较对照组低(P<0.05),治疗2周后,两组患者治疗总有效率之间比较,差异无统计学意义(P>0.05),但观察组患者的治愈率高于对照组(P<0.05);治疗2周后,观察组患者肛门失禁发生率低于对照组(P<0.05);随访3个月两组间复发率比较,差异无统计学意义(P>0.05)。结论:一次性根治术可有效的提高肛周脓肿合并肛瘘的临床治疗效果。

Objective： To analyze the effect of primary radical surgery on perianal abscesses combined with anal fistulas. Methods： 150 patients with perianal abscesses combined with anal fistulas were chosen as the research object and randomly divided into the observation group and the control group,with 75 patients in each group. The observation group were treated with primary radical surgery while the control group were treated with staging drainage,with the clinical efficacy and the recurrence of the two groups analyzed. Results： The postoperative pain time and hospital stay in the observation group were significantly shorter than those in the control group（P〈0.05）,and the cost of hospitalization was significantly lower than that in the control group P〈0.05）. There was no significant difference in the total effective rate between the two groups after 2 weeks of treatment（P〉0. 05）,but the cure rate of the observation group was significantly higher than that of the control group（P〈0.05）. After 2 weeks of treatment,the anal incontinence rate in the observation group was lower than that in the control group,the difference being statistically significant（P〈0.05）. In addition,there was no significant difference in the recurrence rate between the two groups after 3 months follow-up（P〉0.05）. Conclusion： Compared with staging drainage,the primary radical surgery can effectively improve the clinical treatment of perianal abscesses combined with anal fistulas.
