摘要
目的:探讨超声心动图评价妊娠期糖尿病(gestational diabetes mellitus,GDM)患者胎儿左心结构与心室舒张功能。方法:选择159例GDM患者,根据疾病类型分为A组、B组,并选取我院产检正常的孕妇60例作为对照组,均采用彩色B超诊断仪检测胎儿左心结构和心室舒张功能。结果:A组和B组主动脉瓣环内径(aortic annular diameter,AVA)、胎儿二尖瓣(mitral valve,MVA)、左室短轴缩短率(Fraction shortening,LVFS)均高于对照组胎儿(P<0.05),B组胎儿的MVA、AVA、左室壁厚度(Left ventricular wall thickness,LVWT)、LVFS、左室射血分数(left ventricular ejection fraction,LVEF)均高于A组相应指标(P<0.05);A和B组E/A、S/D、Vp均高于对照组(P<0.05),A组胎儿的DT、IVRT均高于B组胎儿水平(P<0.05)。结论:超声心动图可以准确评价妊娠期糖尿病患者胎儿心脏血流传播速度、心室舒张功能及左心结构。
Objective: To explore the echocardiographic evaluation of fetal left heart structures and ventricular diastolic function in gestational diabetes mellitus(GDM). Methods: 159 cases of GDM were chosen and divided into Group A and Group B according to the type of disease,and60 normal pregnant women were chosen as the control group,with the fetal left heart structures and ventricular diastolic function detected by the color Doppler ultrasound. Results: The AVA,PVA and LVFS in Group A and Group B were higher than those in the control group(P〈0.05). The MVA,AVA,LVWT,LVFS and LVEF in Group B were higher than those in Group A. The E/A,S/D and Vp in Group A and Group B were higher than those in the control group. The DT and IVRT in Group A were higher than those in Group B(P〈0.05). Conclusion: Echocardiography can accurately evaluate fetal heart blood flow velocity,ventricular diastolic function and left heart structures in GDM.
