吞咽康复干预对社区脑卒中吞咽障碍的疗效观察

Efficacy of Swallowing Rehabilitation Intervention on Swallowing Disorders in Community Stroke Patients

目的探讨三级康复专科医院吞咽康复指导结合社区康复护理对社区病房脑卒中后吞咽障碍患者吞咽功能恢复的效果。方法选取德胜社区病房2016年2月至2017年10月收治的因脑卒中导致的吞咽障碍患者60例,采用数字随机法分为观察组和对照组,对照组给予常规社区吞咽康复护理,观察组在对照组康复护理的基础上给予三级康复专科医院吞咽康复指导,分别采用洼田饮水试验、Gugging吞咽功能评估量表(GUSS)及标准吞咽功能评估量表(SSA)评估干预前及干预两个月后两组患者的吞咽功能变化情况。结果两组患者洼田饮水试验、GUSS及SSA评估结果干预后均有明显改善(P<0.05);观察组较对照组吞咽功能恢复明显(P<0.05)。结论三级康复专科医院康复指导结合社区康复护理,可有效改善社区病房脑卒中患者的吞咽功能,可显著提高康复治疗效果。

Objective To investigate the effect of swallowing rehabilitation guidance from tertiary rehabilitation specialty hospital combined with community-based rehabilitation nursing in patients with swallowing dysfunction after stroke in community ward. Methods Sixty patients with post-stroke dysphagia admitted to Desheng Community ward from February 2016 to October 2017 were randomly divided into observation group and control group. The control group was given routine community swallowing nursing. The observation group was treated in swallowing rehabilitation training guidance from tertiary rehabilitation specialty hospital on the basis of routine nursing. After two months of intervention,using Watian＇s drinking water test,gugging swallowing screen（ GUSS） and standardized swallowing assessment（ SSA） assessed swallowing function changes between two groups of patients. Results After intervention,The Watian drinking water test,GUSS and SSA assessment results in both groups were significantly changed（ P〈0. 05）,and the recovery of swallowing function in the observation group was significantly better than those in the control group（ P〈0. 05）. Conclusion The rehabilitation guidance from tertiary rehabilitation specialty hospital combined with community-based rehabilitation nursing can effectively improve the swallowing function of patients with swallowing disorder in community ward after stroke, and significantly improve the rehabilitation treatment.
