推拿防治小儿脾肺气虚型反复呼吸道感染的临床观察

Clinical Observation on the Effect of Tuina in Prevention and Treatment of Recurrent Respiratory Infection Induced by Pediatric Lung Qi Deficiency

目的观察推拿法防治小儿脾肺气虚型反复呼吸道感染的临床效果。方法收集肺脾气虚型反复呼吸道感染患儿80例,随机分为治疗组和对照组,每组各40例。治疗组采用推拿疗法,隔日1次,共治疗7次,对照组口服玉屏风散颗粒,每日2次,共服用2周。观察治疗前后2组患儿的总有效率、中医证候评分、血清免疫球蛋白、T细胞亚群以及治疗结束后3月随访复发率等指标,判断推拿疗法的疗效。结果治疗组在总有效率、中医证候评分、IgG、IgA、IgM、CD3^+、CD4^+、CD8^+、CD4^+/CD8^+等免疫指标、复发率等指标上优于对照组,差异具有统计学意义(P<0.05~0.01)。结论小儿推拿可减少肺脾气虚型小儿反复呼吸道感染的发病次数,缓解临床症状,增强患儿体液免疫、细胞免疫功能,降低复发率。

OBJECTIVE To observe the clinical effect of Tuina in prevention and treatment of recurrent respiratory infection induced by pediatric lung qi deficiency.METHODS Eighty children with recurrent respiratory infection induced by lung qi deficiency were collected and were randomly divided into the treatment group and the control group,40 cases in each group.The treatment group was treated with Tuina,one time every other day,and treated for 7 times totally.The control group was intervened by orally taking Yu Ping Feng Powder,2 times/day,2 weeks totally.Indexes including total effective rate,TCM syndrome score,serum immunoglobulin and T lymphocyte subsets before and after the treatment,and recurrence rate during follow-up at 3 months after finish the treatment in both groups were observed for evaluating the curative effect of Tuina.RESULTS The total effective rate,TCM syndrome score,immunological indexes including IgG,IgA,IgM,CD3~＋,CD4~＋,CD8~＋and CD4~＋/CD8~＋,and recurrent rate in the treatment group were better than those of the control group,the differences were statistically significant（P〈0.05,P〈0.01）.CONCLUSION Infantile Tuina can reduce the frequency of pediatric recurrent respiratory infection induced by lung and spleen qi deficiency,relieve clinical symptoms,enhance the humoral immunity and cellular immune function in children,and reduce the recurrence rate.
