超声引导下麦默通微创旋切术在BI-RADS 4级乳腺病灶诊治的应用 被引量:7

摘要 目的分析超声引导下应用麦默通微创旋切系统在BI-RADS 4级乳腺病灶诊治中的价值。方法分析本院于2015年11月至2017年12月确诊的BI-RADS 4级乳腺病灶680例的患者资料,所有患者均于超声引导下应用麦默通微创旋切手术诊治,分析患者术后肿物良恶性情况、病理类型、术后并发症及术后3个月返院行彩超复查时良性肿物是否有残留。结果 380例BI-RADS 4A级乳腺肿物经活检后确诊32例乳腺癌,占8.42%;255例4B级乳腺肿物活检后确诊72例乳腺癌,占28.24%;45例4C级活检确诊35例乳腺癌,占77.78%。乳腺增生和乳腺纤维瘤是最常见的良性肿瘤病理类型,占83.55%,浸润性导管癌是最常见的恶性肿瘤病理类型,占71.23%。皮下淤血是最常见的术后并发症,发生率为23.23%。结论超声引导下应用麦默通微创旋切手术于BI-RADS 4级乳腺病灶诊治中的价值较高,值得临床推广。 Objective To analyze the value of applying ultrasound-guided minimally invasive resection of mammotome surgery on the diagnosis and treatment of BI-RADS 4 mastopathy. Methods 680 cases with BI-RADS 4 mastopathy underwent ultrasound-guided minimally invasive surgery of mammotome from December 2015 to November 2017 were retrospectively analyzed. The patients' pathological results,benign and malignant pathological type,postoperative complications and postoperative tumor residual 3 months after operation were analyzed. Results 380 cases of BI-RADS 4 A breast tumor were diagnosed as 32 cases of breast cancer after biopsy,accounting for 8. 42%,255 cases of 4 B breast tumor were confirmed as 72 cases of breast cancer after biopsy,accounting for28. 24%,45 cases of 4 C breast tumor were confirmed as 35 cases of breast cancer after biopsy,accounting for 77. 78%. Hyperplasia of mammary glands and mammary gland fibroma were the most common pathological types of benign tumors,accounting for 83. 55%,invasive ductal carcinoma was the most common pathological type of malignant tumors,accounting for 71. 23%. Subcutaneous congestion was the most common postoperative complication,the incidence rate of which was 23. 23%. Conclusion Ultrasound-guided minimally invasive resection of mammotome surgery is of high value on the diagnosis and treatment of BI-RADS 4 mastopathy,which is worthy of clinical promotion.
