多焦点人工晶状体植入术后视觉质量的研究 被引量:2

摘要 目的对比研究多焦点与单焦点非球面人工晶状体(intraocular lens,IOL)植入后,对患者视力、对比敏感度及波前相差的影响。方法将白内障并需要手术治疗的患者56例59眼分为多焦点IOL组24例24眼及非球面单焦点IOL组32例35眼。术后1个月检查裸眼远视力(uncorrected distance visual acuity,UDVA)和裸眼近视力(uncorrected near visual acuity,UNVA),测量对比敏感度以及波前像差和点扩散函数(point spread function,PSF)。结果术后1d及1周多焦点IOL组UDVA低于单焦点IOL组;术后1d、1周和1个月多焦点IOL组UNVA优于单焦点IOL组。术后1个月在明视有眩光对比敏感度中,多焦点IOL组在1.5c/d和3.0c/d条件下低于单焦点IOL组。术后1个月在暗视有眩光对比敏感度中,多焦点IOL组在1.5c/d、3.0c/d条件下低于单焦点IOL组。术后1个月在明视无眩光对比敏感度中,多焦点IOL组在1.5c/d和3.0c/d条件下低于单焦点IOL组。术后1个月在暗视无眩光对比敏感度中,多焦点IOL组在6.0c/d条件下敏感度低于单焦点IOL组。多焦点IOL组总像差、总高阶像差、慧差、球差高于单焦点IOL组,差异均有统计学意义(P<0.05)。结论多焦点IOL术后给患者提供了较好的远、近视力,但存在对比敏感度减低的问题。多焦点IOL高阶相差较单焦点IOL有所增加,同时PSF形态表明,眼内2个焦点对成像质量有所影响。
