进展性脑梗死的相关危险因素分析

Exlpore the related risk factors for progressive cerebral infarction

目的探讨影响进展性脑梗死的相关危险因素。方法本研究是回顾性队列研究,收集2015年3月~2017年3月于本院收治的214例急性脑梗死患者的临床资料,按入院后病情进展情况分为进展组与对照组,统计两组临床资料,探讨进展性脑梗死的独立危险因素。结果单因素分析显示糖尿病史、脑卒中史、高血压史(>5年)、高D-二聚体、高同型半胱氨酸、颅内动脉狭窄及同侧颅外段血管狭窄为进展性脑梗死的相关危险因素(P<0.05)。回归分析证实,糖尿病史、颅内动脉狭窄、同侧颅外段血管狭窄、高D-二聚体水平、高血压(>5年)为进展性脑梗死的独立危险因素(P<0.05)。结论糖尿病史、颅内动脉狭窄、同侧颅外段血管狭窄、高D-二聚体水平、高血压(>5年)等因素是诱发脑梗死进展的独立危险因素,积极控制相关危险因素、对优化进展性脑梗死患者生活质量有重要指导作用。

Objective Explore the related risk factors for progressive cerebral infarction. Methods It is a retrospective cohort study, Clinical data of 214 patients with treated in our hospital were enrolled from March 2015 to March 2017. According to the progress of the disease, they were divided into the progress group and the control group. The clinical data of the two groups were analyzed, and then, explore the independent risk factors for progressive cerebral infarction. Results Univariate analysis showed that the indenpendent risk factors for progressive cerebral infarction included diabetes mellitus, prior stroke, hypertension（〉5 y）, high D-dimer level, hyperhomocysteinemia, intracranial artery stenosis together with ipsilateral extracranial artery stenosis（P〈0.05）. Binary regression analysis confirmed that the indenpendent risk factors included diabetes mellitus, hypertension（〉5 y）, high D-dimer level, intracranial artery stenosis together with ipsilateral extracranial artery stenosis（P〈0.05）. Conclusion The independent risk factors for the progressive cerebral infarction were diabetes mellitus, hypertension（5 y）, high D-dimer level, intracranial artery stenosis together with ipsilateral extracranial artery stenosis. It is crucial to control the risk factors actively and optimize the quality of life of patients with progressive cerebral infarction.
