老年骨折患者居家护理服务需求及影响因素的分析

Analysis on the demands and influencing factors of home care service for elderly patients with fracture

目的探讨老年骨折患者的居家护理服务需求及影响因素。方法采用便利抽样方法 ,选取2014年4月~2017年8月罗定市红十字会医院和罗定市人民医院收治的256例老年骨折患者作为研究对象。利用自行设计老年骨折居家护理服务需求调查表和改良Bathel指数评估量表,调查老年骨折患者的居家护理服务需求现状及影响因素。结果本研究中有85.94%(220/256)的患者存在不同程度的日常生活自理能力障碍。不同程度日常生活自理能力患者的生活护理、心理精神、健康指导、安全护理、康复护理及总需求评分比较,差异均有统计学意义(P<0.05)。多因素Logistic回归分析结果提示,文化程度、婚姻状况及家庭人均经济收入是老年骨折患者居家护理服务需求的重要影响因素(P<0.05)。结论罗定地区老年骨折患者的居家护理服务总体需求量大,应根据老年患者的需求制定和提供个性化的居家护理服务,更有利于患者的早日康复,提高患者的生活质量。

Objective To explore the demands and influencing factors of home care service for elderly patients with fracture.Methods With the convenient sampling method,256 cases fracture patients treated in Luoding City Red Cross Hospital and Luoding People′ s Hospital from April 2014 to August 2017 were selected as subjects.The self-designed elderly fracture home care needs questionnaire and modified Bathel index assessment were used to investigate the status of home nursing service demands and influencing factors for elderly patients with fracture.Results In this study,there were 85.94%（220/256） patients with varying degrees of daily self-care disability.The scores of life care,psychological spirit,health guidance,safety nursing,rehabilitation nursing and total demand of patients with different degrees of self-care were compared,and the differences were statistically significant（P〈0.05）.The results of multivariate Logistic regression analysis indicated that the degree of culture,marital status and economic income per capita were the important influencing factors for the demand of home care services for elderly patients with fracture（P〈0.05）.Conclusion There is a large home care services demands of elderly patients with fracture in Luoding area.According to the demands of the elderly patients and provide personalized home care services,it is more conducive to the early recovery of patients and improve the quality of patients′ life.
