成批重度发烟罐烟雾吸入性肺损伤患者的护理

Nursing Care for Severe Patients with Smoke Inhalation Lung Injury by Smoke Pot

目的探讨成批重度发烟罐烟雾吸入性肺损伤患者的护理方法。方法回顾性分析并总结2016年3月某医院救治的5例重度发烟罐烟雾吸入性损伤患者的临床资料。结果 4例患者病情好转,住院时间为96~110d,相关并发症得到有效控制,后期均发生气道黏膜瘢痕增生、气道狭窄及肺纤维化等后遗症。1例患者因多器官功能衰竭并发大咯血抢救无效死亡。结论采取机械控制通气联合体外膜肺氧合、有效排痰、病情监测及早期预防并发症等综合护理措施,能提高发烟罐烟雾吸入性肺损伤患者救治成功率。

Objective To discuss nursing care methods for patients with severe smoke inhalation lung injury by smoke pot.Methods Clinical data of five cases of severe smoke inhalation lung injury in the 180 th Hospital of People＇s Liberation Army were analyzed retrospectively.Results Four patients were discharged after 96 to 110 days,and the related complications were effectively controlled,but they suffered the sequelae of airway mucosal scarring,airway stenosis and pulmonary fibrosis in the later period.One patient died from multiple organ failure and hemoptysis.Conclusions Taking comprehensive nursing measures such as controlled mechanical ventilation combined with extracorporeal membrane oxygenation,effective drainage,condition monitoring and early prevention complications can improve the success rate of treatment.
