CT灌注成像技术在颅脑创伤周围缺血病灶的应用

摘要 目的探讨CT灌注成像(CTP)技术指导治疗颅脑创伤周围缺血病灶的临床意义。方法对头颅CT提示颅脑创伤病灶周围低密度影的患者行CTP检查,选取低灌注患者随机分为对照组和试验组。对照组予常规治疗,试验组在常规治疗的基础上增加血容量并改善微循环;通过格拉斯哥预后评分(GOS评分)、简明精神状况检查量表(MMSE)、蒙特利尔认知评估量表(MoCA)对比两组预后及认识水平。结果两组治疗后GOS评分、MMSE评分及MoCA差异均有统计学意义(均P<0.05)。结论对于颅脑创伤周围缺血病灶患者早期增加血容量并改善微循环可增加该病灶区域的血流灌注,改善认知功能。CTP技术可以为临床指导治疗颅脑创伤周围缺血病灶提供可靠的影像学依据。 Objective To discuss the clinical significance ofCT perfusion (CTP) technique in the treatment of peri- pheral ischemic lesions of eraniocerebral trauma. Methods CTP examination was performed on craniocerebral tra- uma patients whose CT indicated low-density shadow around the trauma lesion. Patients with hypoperfusion were randomly divided into control and experimental groups. The control group received routine treatment. The experi- mental group received treatment of increasing blood volume and improving microcirculation on the basis of routine treatment. The prognoses of the two groups were evaluated and compared by the Glasgow outcome scale (GOS), mini-mental state examination (MMSE) and Montreal Cognitive Assessment (MoCA). Results There were statis- tical significances of GOS score, MMSE score and MoCA between the two groups after treatment ( all P 〈 0.05). Conclusions As for craniocerebral trauma patients with peripheral ischemia, increasing blood volume and impro- ving microcirculation at the early stage can increase blood perfusion in the lesion area and improve cognitive function. CTP can provide a reliable imaging basis for the clinical guidance of peripheral ischemic lesions.
