摘要
目的探寻该地区纺织印染行业不同工种和各种职业危害因素与鼻咽癌发病风险之间的关系。方法采用队列研究方法,应用Cox比率风险模型,通过本地印染企业员工职业卫生档案随机抽取组成的研究队列(n=3 176),与2002—2016年间确诊的81份NPC病例进行对比研究。结果 (1)皮毛处理、加工(95%CI:0.433-13.419)、印染(95%CI:1.306-26.334)、质量检验、仓库、包装(95%CI:0.327-11.605)从业>10年NPC风险增加最为显著;(2)化纤(HR*=1.525,95%CI:0.376-6.189)、腐蚀性酸剂(HR*=1.956,95%CI:0.602-6.356)2个职业暴露危险因素的NPC发病风险率最高;(3)化纤、腐蚀性酸剂及内毒素3个危险因素暴露时间增加,其NPC风险增加最为显著。结论从事印染、原材料处理、加工、库管质检等工种,以及有化纤、腐蚀性酸剂、内毒素等暴露因素的产业工人随着接触时间的增加患NPC风险显著增高。
Objective To explore the relationship between different occupational hazards and the risk of nasopharyngeal cancer in the textile and dyeing industry in this region. Methods Queue and risk ratio of Cox model was used,through the local printing and dyeing enterprise staff's occupational health archive,the randomly selected study cohort( n = 3 176) was set up,and the comparison of the diagnosed NPC cases between 2002 and 2016 was performed. Results 1. The fur processing,processing( 95% CI: 0. 433-13. 419),printing and dyeing( 95% CI: 1. 306-26. 334),quality inspection,storage,packaging( 95% CI: 0. 327-11. 605),engaged in those professions for 10 years,had the highest incidence of NPC risk. 2. The professional exposure of chemical fiber( HR*= 1. 525,95% CI: 0. 376-6. 189),and corrosive acid( HR*= 1. 956,95% CI: 0. 602-6. 356) had the highest NPC risk. 3.Due to the increased exposure time of the three risk factors,such as chemical fiber,corrosive acid and endotoxin,the increase of NPC risk was most significant. Conclusion Those workers engaged in printing and dyeing,material handling,machining,library tube quality inspection work,and chemical fiber,acid corrosion,endotoxin,their risk of NPC was significantly higher. with the increase of exposure time.
