经皮穴位电刺激配合基础康复训练治疗脑卒中后手功能障碍临床研究

Clinical Study of TEAS Coordinated with Basic Rehabilitation Training on Hand Dysfunction after Stroke

目的:探讨经皮穴位电刺激配合基础康复训练对脑卒中后手功能障碍的康复效果。方法:将108例脑卒中后手功能障碍患者随机分为两组,对照组(54例)采用基础康复训练治疗,研究组(54例)在基础康复治疗同时对患肢进行经皮穴位电刺激治疗,疗程为12周。比较两组治疗前后Brunnstrom分级、腕关节功能活动度、手指功能和日常生活活动能力。结果:研究组治疗12周时掌曲角度优于对照组(P<0.05),4周、12周时背伸角度优于对照组(P<0.01);治疗4周时研究组简化Fugl-Meyer评定量表(FBA)评分高于对照组(P<0.01),治疗12周时研究组FBA、改良Bathel指数(MBI)评分均高于对照组(P<0.01),治疗后研究组手Brunnstrom分级优于对照组(P<0.05),差异均有统计学意义。结论:经皮穴位电刺激配合基础康复训练可有效增加腕关节背伸、掌曲角度,改善手指运动功能、生活活动能力和患侧手Brunnstrom分级。

Objective： To investigate the rehabilitation effects of transcutaneous electrical acupoint stimulation（ TEAS） coordinated with basic rehabilitation training on post-stroke hand dysfunction.Methods： 108 patients with hand dysfunction after stroke were randomly divided into the control group（ n = 54） and the study group（ n = 54）.The control group was treated with basic rehabilitation treatment; on which basis,the study group was also treated with TEAS on affected limb.The course of the treatment was 12 weeks.Brunnstrom stages,motion range of wrist joint function,finger function and activities of daily living were compared before and after the treatment in the two groups.Results： The palm curvature was significantly better in the study group than that in the control group after 12 weeks of the treatment（ P〈0.05）; dorsiflexion of the study group was superior to that of the control group after 4 weeks and 12 weeks（ P 0.01） of the treatment; scores of simplified Fugl-Meyer rating scale was greatly higher in the study group than that in the control group after 4 weeks and 12 weeks of the treatment（ P 0.01）; score of modified Bathel index was also significantly higher in the study group than that in the control group after 12 weeks of the treatment（ P 0.01）; the Brunnstrom stages of hand was better than that of the control group after the treatment（ P 0.05）.Conclusion： TEAS coordianted with basic rehabilitation training can effectively increase palm curvature and dorsiflexion,it can improve the function of finger movement,activities of daily living and Brunnstrom stages of affected hand.
