双心模式干预在慢性心衰伴抑郁障碍患者中的相关研究

Double Heart Mode Intervention in Patients With Chronic Heart Failure and Depression

目的针对双心模式干预在慢性心衰伴抑郁障碍患者中相关研究的前瞻性和回顾性临床数据进行分析,使读者在对心衰且伴发心理障碍患者的生存现况有部分了解的前提下,充分意识到影响心衰患者治愈后的两个重要因素分别是焦虑和抑郁治疗。方法对入院时间达到24 h内的患者进行干预前评分,采用PHQ-2和PHQ-9两类健康调查问卷,将评分大于5分的患者纳入研究,入选后随机分为两组:常规治疗组和双心模式干预组,两组均给予常规硝酸酯扩冠、强心利尿治疗。结果通过对比实验认识慢性心衰伴抑郁障碍患者经双心模式治疗前后血清炎性因子(IL-6、TNF-α、CRP、Hcy、BNP、Cys C)的变化以及抑郁情绪对慢性心衰患者血清炎性因子(IL-6、TNF-α、CRP、Hcy、BNP、Cys C)水平的影响。治疗3个月后,双心模式干预组LVEF、BNP、CRP改善情况优于常规治疗组,差异均具有统计学意义(P<0.05)。结论经双心模式治疗后,心衰合并抑郁患者可明显改善心功能,改善CRP、BNP、IL-6、TNF-α、Cys C、Hcy,提高LVEF%,达到心血管和心理双重治疗,改善患者预后,促进康复。

Objective The prospective and retrospective clinical data of two heart mode interventions in patients with chronic heart failure and depressive disorder were analyzed. To enable readers to have a partial understanding of the survival status of patients with heart failure and associated mental disorders, it is fully realized that the two important factors influencing the cure of patients with heart failure are anxiety and depression treatment. Methods The patients who had the time of admission to 24 h were scored before the intervention, and the two types of health questionnaires were adopted, and the patients with the score of more than 5 were included in the study. After being selected, they were randomly divided into two groups： the conventional treatment group and the double heart mode intervention group. The two groups were given conventional nitrite dilation and cardio diuresis treatment. Results The changes of serum inflammatory factors（IL-6, TNF-a, CRP, Hcy, BNP, Cys C） in patients with chronic heart failure and depressive disorder were studied through comparative experiments and the effects of depression on the levels of serum inflammatory factors（IL-6, TNF-a, CRP, Hcy, BNP, Cys C） in patients with chronic heart failure. After 3 months of treatment, the improvement of LVEF, BNP and CRP in the two heart mode intervention group was better than that in the conventional treatment group, and the difference was statistically significant（P〈0.05）. Conclusion After the treatment of double heart mode, heart failure patients with depression can obviously improve heart function, improve CRP, BNP, IL-6, TNF-alpha, Cys C, Hcy, improve LVEF%, achieve dual cardiovascular and psychological treatment, improve the prognosis of patients and promote rehabilitation.
