新型抗凝药利伐沙班联合护理干预用于关节置换术对深静脉血栓的预防作用

Effect of new anticoagulant drug rivaroxaban combined with nursing intervention for preventing deep venous thrombosis of joint replacement surgery

目的研究新型抗凝药利伐沙班联合护理用于关节置换术对深静脉血栓(DVT)的预防作用。方法收集我院2015年8月至2017年9月收治的68例接受关节置换术治疗的患者作为研究对象,术后均给予新型抗凝药利伐沙班治疗,在此基础上按数字奇偶法分为试验组与对照组,各34例。对照组联合常规护理干预,试验组则联合早期护理干预。观察比较两组的预防效果情况,测定并分析两组干预前、后凝血指标(APTT、PT、FIB)及D-D情况。结果试验组DVT发生率明显低于对照组,差异具有统计学意义(P<0.05)。干预后,两组APTT及PT均上升,且试验组显著高于对照组(P<0.05),两组FIB、D-D水平均明显降低,且试验组明显低于对照组,差异具有统计学意义(P<0.05)。试验组的护理满意度显著高于对照组,差异具有统计学意义(P<0.05)。结论针对关节置换术患者,术后给予新型抗凝药利伐沙班治疗的同时联合早期护理干预能够显著改善患者的血液黏稠度,预防DVT的发生,提高患者的护理满意度。

Objective To study the effect of new anticoagulant drug rivaroxaban combined with nursing intervention forpreventing deep venous thrombosis （DVT） of joint replacement surgery. Methods A total of 68 cases of patients with jointreplacement surgery in our hospital from August 2015 to September 2017 were collected as the research objects and givennew anticoagulant drug rivaroxaban after surgery. All the patients were divided into experimental group and control groupaccording to digital parity method based on this treatment, with 34 cases in each group. The control group received routinenursing intervention, while the experimental group received early nursing intervention. The preventive effects of the twogroups were observed and compared, the coagulation indexes （APTT, PT, FIB） and D-D levels before and after interventionin both groups were measured and analyzed. Results The incidence of DVT in the experimental group was significantlylower than that in the control group（P〈0.05）. After the intervention, APTT and PT increased in both groups increased, andthose of the experimental group were significantly higher than the control group （P〈0.05）; the FIB and D-D levels in thetwo groups significantly decreased, and those of the experimental group were significantly lower than the control group, thedifferences were statistically significant （P〈0.05）. The rate of nursing satisfaction in the experimental group was significantlyhigher than that in the control group, the difference was statistically significant（P〈0.05）. Conclusion For patients with jointreplacement, using new anticoagulant drug rivaroxaban combined with nursing intervention after surgery can significantlyimprove the blood viscosity of patients, prevent the occurrence of DVT and improve patients＇ nursing satisfaction.
