α-甘露聚糖肽治疗恶性胸腔积液的临床观察

摘要 目的:观察胸腹腔内注射α-甘露聚糖肽治疗恶性胸腹水的疗效和副作用。方法:选择2012年12月~2015年6月收治的恶性胸腔积液患者45例,随机分为对照组和联合组,对照组23例,尽量抽尽胸腔积液后,胸腔内注射顺铂(DDP)60~80 mg/次;联合组22例,在进行上述常规治疗的同时,加用α-甘露聚糖肽20 mg/次胸腔内注射,每周1~2次,连续2周为1个疗程。结果:联合组胸水吸收的有效率优于对照组;联合组发热发生率高于对照组,联合组不良反应率高于对照组,差异均有统计学意义(P<0.05);两组均未见肝、肾功能毒性反应。结论:α-甘露聚糖肽与顺铂联合应用治疗恶性胸腔积液疗效突出,临床应用耐受性良好,值得进一步应用。 Objective:To evaluate the therapeutic effects of mannatide in the treatment of malignant pleural effusion. Methods:From December 2012 to June 2015,64 cases with malignant effusions were randomly classified into two groups. 22 cases in the combination group were treated with Mannatide plus DDP intrapleural injection. while 23 cases in the control group were with DDP intrapleural injection. A dose of 20 mg of Mannatide and 60 ~ 80 mg of DDP was injected intrapleurally to the patients with malignant pleural effusion respectively in the combination group. The same dose of DDP was injected intrapleurally only to the patients with malignant pleural effusion in the control group. The above dosage was given 1-2 times/w for 2 weeks. Finally the effect and side effects were observed. Results: The effective rates of malignant pleural effusion in the combination group were superior to those in the control group. The incidence of fever was higher in the combined group than in the control group,and the rate of adverse reactions was higher in the control group. There was statistically significant difference( P〈 0. 05). The chief side effect was pyrexia and no toxicity was found on blood picture liver or kidney functions. Conclusions:Recombinant treatment of malignant pleural effusion by blood stream-mannose polysaccharide( Lifein) and cisplatin is effective. The clinical application has good tolerance,worthy of further application.
