类风湿关节炎患者关节滑膜液miR-203和MMP-3表达及其临床意义

The expression and clinical significance of miR-203 and MMP-3 in synovial fluid of patients with rheumatoid arthritis

目的探讨类风湿关节炎(RA)患者关节滑膜液中miR-203和MMP-3表达的相关性及其临床意义。方法选取RA患者60例作为研究组,选取我院健康志愿者30例作为对照组,qRT-PCR检测miR-203表达水平,ELISA法检测MMP-3表达水平。ROC曲线分析评价其对RA诊断的灵敏度及特异度。结果 RA患者关节滑膜液中miR-203和MMP-3表达高于对照组,其在活动期表达升高更明显,两者存在显著正相关(r=0.678,P<0.001)。相关性分析显示,miR-203、MMP-3与RA临床指标RF、ACCP、CRP、ESR及DAS28等存在正相关性。ROC曲线分析表明关节滑膜液miR-203及MMP-3对RA具有较高的诊断灵敏度和特异度,联合检测能提高疾病的临床诊断能力。结论miR-203及MMP-3在患者关节滑膜液中存在差异表达,这种差异具有较强的临床诊断能力,有望成为新的临床诊断指标;通过调控miR-203及MMP-3表达,可以作为RA潜在治疗靶点。

Objective To investigate the expression and clinical significance of miR-203 and MMP-3 in synovial fluid of patients with rheumatoid arthritis （RA）. Methods Sixty RA patients were selected as the study group, and 30 healthy volunteers in our hospital were selected as healthy control group. The expression of miR-203 was detected by qRT-PCR, and the expression of MMP-3 by ELISA. ROC curve analysis was used to evaluate the sensitivity and specificity of RA diagnosis. Results The expression of miR-203 and MMP-3 in RA patients was higher than that in the control group, and the expression increased significantly in the active phase, and there was a significant positive correlation between them （r=0.678, P〈0.001）. Correlation analysis showed that there was a positive correlation between miR-203, MMP-3 and RK ACCP, CRP, ESR and DAS28. ROC curve analysis showed that miR-203 and MMP-3 had high diagnostic sensitivity and specificity for RA, and combined detection could improve the clinical diagnostic ability of the disease. Conclusion The differential expression of miR-203 and MMP-3 in the synovial fluid of patients with RA have a strong clinical diagnostic ability, are expected to become a new clinical diagnostic indicator, and can be used as RA potential therapeutic target by regulating the expression of miR-203 and MMP-3.
