健骨汤对经皮椎体成形术后患者骨代谢及骨密度的影响 被引量:5

Effect of Jiangu Decoction on Bone Metabolism and Bone Mineral Density in Patients Undergoing Percutaneous Vertebroplasty

摘要 目的探讨健骨汤对经皮椎体成形术(PVP)后患者骨代谢及骨密度的影响。方法选择医院2015年1月至2017年1月收治的骨质疏松性胸腰椎压缩骨折老年患者89例,随机分为对照组44例和治疗组45例。对照组患者给予PVP术后常规治疗,治疗组患者给予PVP术后健骨汤治疗,两组患者均连续治疗3个月。结果治疗组的总有效率为95.56%,明显高于对照组的79.55%(χ~2=5.264,P=0.022);治疗后,治疗组患者Cobb角和椎体骨密度均明显优于对照组(P<0.05),血清骨钙素(BGP)、骨碱性磷酸酶(BALP)及Ⅰ型前胶原羧基端肽(PICP)水平均明显高于对照组(P<0.05);新发骨折、椎旁血管渗漏、椎旁软组织渗漏、椎间隙渗漏的发生率均明显低于对照组(P<0.05)。结论健骨汤联合PVP治疗骨质疏松性胸腰椎压缩骨折的临床疗效显著,可恢复伤椎解剖结构,增加骨密度,改善骨代谢,且能降低并发症发生率,值得推广。 Objective To investigate the effect of Jiangu Decoction on bone metabolism and bone mineral density in patients undergoing percutaneous vertebroplasty( PVP). Methods Totally 89 elderly patients with osteoporotic thoracolumbar vertebral compression fracture admitted to our hospital from January 2015 to January 2017 were selected and randomly divided into the control group( n = 44) and the treatment group( n = 45). The control group was treated with conventional treatment after PVP,while the treatment group was treated with Jiangu Decoction after PVP. The two groups were treated for 3 months. Results The total effective rate of the treatment group was95. 56%,which was significantly higher than 79. 55% of the control group( χ^2 5. 264, P = 0. 022). After treatment,the Cobb angle and the bone mineral density of injured vertebrae of the treatment group were significantly better than those of the control group( P 0. 05). The levels of serum BGP,BALP and PICP in the treatment group were significantly higher than those in the control group( P 0. 05). The incidence rates of complications such as new fracture,vertebral side vascular leakage,vertebral side soft tissue leakage and intervertebral space leakage in the treatment group were significantly lower than those in the control group( P 0. 05). Conclusion Jiangu Decoction combined with PVP for treating elderly patients with osteoporotic thoracolumbar vertebral compression fracture has a good effect and low adverse reactions, it can adjust the anatomical structure of injured vertebrae, improve the bone mineral density and bone metabolism,and reduce the incidence of complications,which is worthy of promotion.
