克拉霉素联合呋喃唑酮对老年胃溃疡复发率及生活质量的影响

Effect of Clarithromycin Combined with Furazolidone on Recurrence Rate and Quality of Life in Elderly Patients with Gastric Ulcer

目的探讨克拉霉素联合呋喃唑酮对老年胃溃疡临床疗效及复发率、生活质量的影响。方法选取医院住院部及门诊2015年1月至2016年1月收治的老年胃溃疡患者240例,根据治疗方案不同分为研究组和对照组,各120例。对照组患者给予奥美拉唑+呋喃唑酮治疗,研究组患者在此基础上加用克拉霉素治疗,均治疗14 d。结果研究组临床总有效率为90.83%,高于对照组的77.50%(P<0.05);治疗后,两组患者腹痛腹胀、恶心呕吐、嗳气反酸评分均明显降低(P<0.05),且研究组患者症状评分降低幅度明显高于对照组(P<0.05);研究组患者治疗结束后幽门螺杆菌根除率为84.17%,显著高于对照组的69.17%(P<0.05),且1年内复发率为12.50%,显著低于对照组的24.17%(P<0.05);研究组患者治疗结束后3个月简明健康状况量表-36(SF-36)评分中生理功能、生理职能、活力、精神健康、躯体疼痛及总体健康维度评分均显著高于对照组(P<0.05)。结论克拉霉素联合呋喃唑酮治疗老年胃溃疡,可提高临床疗效,改善临床症状,减少复发,可有效改善患者生活质量。

Objective To investigate the effect of clarithromycin combined with furazolidone on the recurrence rate and quality of life in elderly patients with gastric ulcer. Methods Totally 240 elderly patients with gastric ulcer admitted to the Department of Inpatient and Outpatient in our hospital from January 2015 to January 2016 were selected and randomly divided into the study group and the control group according to different treatment regimens,120 cases in each group. The control group was treated with omeprazole combined with furazolidone, on this basis, the study group was additionally treated with clarithromycin. The two groups were treated for 14 d. Results The total effective rate of the study group was 90. 83%,which was significantly higher than 77. 50% of the control group（ P 0. 05）.After treatment, the scores of abdominal pain and abdominal distention, nausea and vomiting, belching and acid reflux in the two groups were significantly decreased（ P 0. 05）,and the decrease in the study group was more obvious than that in the control group（ P 0. 05）.The HP eradication rate of the study group was 84. 17%,which was significantly higher than 69. 17% of the control group（ P 0. 05）,and the recurrence rate within 1 year after treatment of the study group was 2. 50%,which was significantly lower than 24. 17% of the control group（ P 0. 05）. The scores of physiological function,physiological role,vitality,mental health,body pain and general health in the 36-item Short-Form Health Survey Questionnaire（ SF-36） in the study group were significantly higher than those in the control group in the third month after treatment（ P 0. 05）. Conclusion Clarithromycin combined with furazolidone in the treatment of elderly patients with gastric ulcer can improve the clinical effect,relieve clinical symptoms and reduce recurrence,it can effectively improve the quality of life of patients.
