中枢神经系统铁沉积症合并椎管内肿物1例

A case of superficial siderosis of the central nervous system with intraspinal mass

目的探讨中枢神经系统铁沉积症合并腰椎肿物的病因、症状特点和诊治要点,加深对此类疾病认识,提高诊断。方法介绍中枢神经系统铁沉积症合并腰椎肿物1例,对其可能病因、症状特点、影像学特点、诊断治疗、预后进行分析。结果患者有感音性听力下降、锥体束征,颅脑MRI示脑池及脑沟周围脑表面低信号,腰椎MRI示腰椎肿物。结论对有感音性听力下降、锥体束征等症状体征患者早期完善头颅MRI明确中枢神经系统铁沉积症诊断,并积极寻找病因,对症治疗,改善患者预后。

Objective To investigate the etiology,clinical features,diagnosis and treatment of superficical siderosis of the central nervous system（SSCNS）with intraspinal mass,deepen reorganization and improve diagnosis level.Methods One case of the SSCNS with intraspinal mass was reported and the possible etiology,clinical features,imaging features,diagnosis,treatment and prognosis were analyzed.Results A 38-year-old male patient with SSCNS had sensorineural hearing impairment and pyramidal sign.Brain MRI demonstrated linear hypointensity signal around the cerebral surface and sulcus,and lumbar MRI showed intraspinal mass.Conclusion Patient with sensorineural hearing loss and pyramidal sign should be examined by brain MRI scan to definite SSCNS diagnosis,and to positively find the cause and provide symptomatic treatment to improve the prognosis.
