HPV16/18型病毒检测与宫颈癌的相关性研究

Correlation between Detection of HPV16/18 Virus and Cervical Cancer

目的:探究HPV16/18型病毒感染与宫颈癌的相关性,分析其临床应用价值。方法:选取2014年1月-2017年1月本院收治的宫颈癌患者90例为观察组,同期来院体检健康者90例为对照组。所有研究对象均采用罗氏Cobas HPV与实时荧光定量PCR检测法测定HPV16/18DNA病毒载量,比较两组的阳性率。结果:观察组HPV16/18NDA载量水平及阳性率均高于对照组,差异有统计学意义(P<0.05);ROC曲线绘制及Logistics多因素回归分析结果显示,HPV16/18DNA载量水平≥(5.56±1.04)、(5.55±0.06)copies/m L为宫颈鳞癌的独立风险因素(HR=1.004、0.783,P=0.045、0.031)。结论:宫颈鳞癌的发病与HPV16/18型病毒的感染具有一定相关性,建议临床完善高危人群的宫颈癌筛查工作,实现早发现、早诊治的目的,同时提示在宫颈癌癌前病变的过程中注意HPV感染。

Objective：To explore the correlation between HPV16/18 virus infection and cervical cancer and analyze its clinical application value.Method：A total of 90 patients with cervical cancer admitted to our hospital from January 2014 to January 2017 were selected as observation group,and 90 healthy people during the same period were selected as control group.Roche Cobas HPV and real-time fluorescent quantitative PCR assay were used to determine the HPV16/18 DNA load of two groups,and the positive rates of two groups were compared.Result：The levels of HPV16/18 NDA load and positive rate in observation group were higher than those of control group,the differences were statistically significant（P〈0.05）.ROC curve plotting and Logistics multivariate regression analysis showed that the levels of HPV16/18 DNA load≥（5.56±1.04） and（5.55±0.06） copies/m L as an independent risk factor for cervical squamous cell carcinoma（HR=1.004,0.783,P=0.045,0.031）.Conclusion：The incidence of cervical squamous cell carcinoma is associated with the HPV16/18 virus infection.It is recommended to improve the screening of cervical cancer in high-risk groups to achieve the purpose of early detection and early diagnosis and treatment,and should pay attention to HPV infection in the process of cervical cancer precancerous lesions.
