围术期应用白蛋白对术前无低蛋白血症结直肠癌术后并发症的影响

Effect of perioperative albumin on postoperative complications of colorectal cancer without preoperative hypoproteinemia

目的探讨围术期应用白蛋白对术前无低蛋白血症结直肠癌术后并发症的影响。方法回顾性分析223例术前无低蛋白血症结直肠癌患者临床资料,按围术期营养支持方式分为对照组112例与白蛋白组111例,对照组围术期采用常规营养支持,白蛋白组在对照组基础上加用人血白蛋白。比较两组患者术后并发症发生情况。结果两组患者术后低蛋白血症、吻合口瘘、肠梗阻、切口水肿发生率比较,差异均无统计学意义(均P>0.05),白蛋白组术后切口感染及总并发症发生率均高于对照组(均P<0.05)。结论术前无低蛋白血症的结直肠癌患者围术期使用人血白蛋白会增加术后切口感染发生的风险。

Objective To explore the effect of perioperative albumin on postoperative complications of colorectal cancer without preoperative hypoproteinemia. Methods The clinical data of 223 patients with colorectal cancer without preoperative hypoproteinemia were retrospectively analyzed.According to perioperative nutritional support approach ,the patients were divided into 112 cases in the control group and 111 cases in the albumin group.The control group received conventional nutritional support in the perioperative period,while the albumin group was given human albumin additionally on the basis of therapy in the control group.The incidence of postoperative complications was compared between the two groups. Results There was no significant difference in the incidence rate of postoperative hypoproteinemia ,anastomotic fistula,intestinal obstruction or incision edema between the two groups（all P 〉0.05）.The incidence rates of postoperative incision infection and overall complications in the albumin group were higher than those in the control group（all P 〈0.05）. Conclusion Perioperative use of human albumin might increase the risk of postoperative incision infection in colorectal cancer patients without preoperative hypoproteinemia.
