妊娠期糖尿病患者产后血清视黄醇结合蛋白4、转化生长因子β_1水平及临床意义

Puerperal levels and clinical significance of serum retinol binding protein 4 and transforming growth factor beta 1 in patients with gestational diabetes mellitus

目的探讨妊娠期糖尿病(GDM)患者产后血清视黄醇结合蛋白4(RBP4)、转化生长因子β1(TGF-β1)水平及其临床意义。方法纳入GDM患者61例(观察组)及正常妊娠产妇50例(对照组)。检测两组产后2个月及4个月的血清RBP4及TGF-β1水平,以及FBG、空腹血胰岛素(FINS)水平,计算稳态模型评估-胰岛素抵抗指数(HOMA-IR)。结果观察组产后2个月、4个月时血清RBP4、TGF-β1水平及FBG、HOMA-IR均高于对照组(P<0.05),两组FINS水平比较差异无统计学意义(P>0.05)。GDM患者产后血清RBP4、TGF-β1水平与FBG、HOMA-IR均呈正相关,血清RBP4与TGF-β1水平呈正相关(P<0.05)。结论 GDM患者产后血清RBP4、TGF-β1水平明显升高,两者均可作为GDM患者产后糖代谢异常的有效监测指标。

Objective To explore the puerperal levels and clinical significance of serum retinol binding protein 4（RBP4） and transforming growth factor beta 1（TGF-β 1） in the patients with gestational diabetes mellitus（GDM）. Methods Sixty-one patients with GDM（observation group） and 50 normal pregnant women（control group） were enrolled.Serum levels of RBP4 and TGF-β 1 at 2 and 4 months after delivery as well as the FBG and fasting insulin（FINS） levels were detected in both groups.Homeostasis Model Assessment of Insulin Resistance（HOMA-IR） index was counted. Results Compared to the control group,the observation group obtained higher levels of serum RBP4 and TGF-β 1 at 2 and 4 months after delivery as well as higher FBG and HOMA-IR（ P 〈0.05）.There was no significant difference in the FINS level between the two groups（ P 〉0.05）.In the patients with GDM,the puerperal levels of serum RBP4 and TGF-β 1 positively correlated with FBG and HOMA-IR,and serum RBP4 level positively correlated with serum TGF-β 1 level（ P 〈0.05）. Conclusion Serum levels of RBP4 and TGF-β 1 increase significantly in the patients with GDM after delivery,and can be the effective indices for monitoring the puerperal glycometabolism abnormality in patients with GDM.
