摘要 目的探究CT血管成像(CTA)在主动脉病变诊断中的应用价值。方法选取在我院就医疑诊有主动脉病变的46例患者作为研究对象,对患者进行CTA检测,对感兴趣区域进行CT增强扫描,采用多平面重建成像(MPR)、最大密度投影(MIP)、曲面重建(CPR)以及容积再现(VR)对所有图像进行重建分析。结果 CT检测显示,46例患者中,主动脉夹层动脉瘤21例,其中De BakeyⅠ型8例、Ⅱ型3例、Ⅲ型10例;主动脉瘤12例,其中主动脉壁内血肿6例、主动脉狭窄2例、主动脉硬化性溃疡4例,正常2例。以手术、数字减影血管造影检测结果为标准,CTA诊断主动脉病变的灵敏度、特异度均为100%。结论 CTA结合三维重建技术能够清晰显示主动脉病变特征、累及范围、病灶大小、夹层动脉瘤真假腔、附壁血栓以及内膜片等,可为临床治疗提供更多信息,具有较高的临床应用价值。 Objective To explore the application value of CT angiography( CTA) in the diagnosis of aortic diseases.Methods 46 patients with suspected aortic diseases were selected as the subjects. Who were detected by CTA,and the region of interest was enhanced by CT enhancement. Multiplane reconstruction imaging( MPR),maximum density projection( MIP),surface reconstruction( CPR) and volume rendering( VR) were used to reconstruct all the images. Results 21 of 46 patients had aortic dissection,including 8 cases of De Bakey I type,3 cases of II type and 10 cases of III type in CT examination results. There were12 cases of aortic aneurysm,including 6 cases of intramural aortic hematoma,2 cases of aortic stenosis,4 cases of sclerosing ulcer of aorta,and 2 cases of normal aortic aneurysm. The sensitivity and specificity of CTA in diagnosing aortic diseases were 100%,which were compared with those of operation and digital subtraction angiography. Conclusion CTA combined with three-dimensional reconstruction technique can clearly display the characteristics of the aortic disease,the extent of involvement,the size of the focus,the true and false lumen of dissecting aneurysm,the thrombus of the wall and the endometrium and so on. It can provide more information for clinical treatment and has a high clinical value.
