腹腔镜下结直肠癌手术的临床疗效观察及对胃肠功能的影响分析

Clinical efficacy of laparoscopic surgery for colorectal cancer and its influence on gastrointestinal function

目的探讨腹腔镜下结直肠癌手术的临床疗效及对胃肠功能的影响。方法选择2013年4月~2016年3月在我院实施结直肠癌手术的患者82例,根据患者及其家属对治疗的意愿将其分为对照组和研究组,对照组41例行传统开放式手术,研究组41例行微创腹腔镜手术,观察比较两组患者的手术时间、术中失血量、住院时间以及治疗前后胃泌素与胃动素水平、术后肠鸣音恢复时间、排气时间、术后并发症发生情况以及1年生存率。结果研究组患者术中失血量与住院时间明显低于对照组,差异有统计学意义(P<0.05)。治疗后,研究组患者的胃动素与胃泌素均高于对照组(均P<0.05),肠鸣音恢复时间及排气时间均低于对照组(均P<0.05);术后并发症发生率比较,研究组为4.88%明显低于对照组的19.51%,差异有统计学意义(P<0.05)。结论腹腔镜下行结直肠癌手术可有效促进术后胃肠功能恢复,提高治疗效果,且并发症少,值得临床应用。

Objective To explore the clinical effect of laparoscopic and rectal cancer surgery and its effect on gastrointestinal function. Methods Choose the April 2013~2016 year in March in our hospital 82 cases of patients with colorectal cancer patients and their families,according to the wishes of the treatment will be divided into two groups：control group（traditional open surgery）and study group（41 cases treated by minimally invasive laparoscopic surgery in 41 cases）.Comparison of two groups of operation time,intraoperative blood loss,hospitalization time,before and after the treatment of gastrin and motilin levels,postoperative recovery time of bowel sound,exhaust time,postoperative complications,and 1 year survival rate. Results The intraoperative blood loss and hospital stay in the study group were lower than those in the control group（P〈0.05）.The levels of motilin and gastrin in the study group were higher than those in the control group（P〈0.05）.The research group,the recovery time of bowel sound and exhaust time lower than that of the control group（P〈0.05）.The incidence of postoperative complications in the study group was 4.88%,which was lower than that of the control group（19.51%）（P〈0.05）. Conclusion Laparoscopic colorectal cancer surgery can effectively ensure the recovery of gastrointestinal function in patients,safe and reliable,suitable for clinical application.
