沙培林胸膜固定术治疗Ⅱ、Ⅲ期矽肺合并反复自发性气胸疗效分析

Efficacy analysis of pleurodesis with Sapylin in Stage Ⅱ and Stage Ⅲ of silicosis with recurrent spontaneous pneumothorax

目的探讨沙培林胸膜固定术治疗Ⅱ、Ⅲ期矽肺合并反复自发性气胸的疗效。方法选取我院住院及门诊随访的Ⅱ、Ⅲ期矽肺合并反复自发性气胸患者78例,随机分为观察组和对照1组、对照2组各26例,3组患者均采用常规胸腔闭式引流术,观察组在胸腔闭式肺复张后给予沙培林胸腔内注射,对照1组在胸腔闭式引流肺复张后给予50%高糖胸腔内注药,对照2组仅行单纯胸腔闭式引流术,观察比较3组患者自发性气胸复发情况。结果 3组患者治疗后3、6、12个月的自发性气胸复发率比较,差异无统计学意义(P>0.05)。结论沙培林胸膜固定术治疗Ⅱ、Ⅲ期矽肺合并反复自发性气胸患者的疗效与高糖胸膜固定术及单纯胸腔闭式引流术的疗效相当。

Objective To analyze the efficacy of pleurodesis with Sapylin in Stage Ⅱ and Stage Ⅲ of silicosis with recurrent spontaneous pneumothorax. Methods 78 Stage Ⅱ and Stage Ⅲ of silicosis patients with recurrent spontaneous pneumothorax in our inpatient department and outpatient department were selected.The subjects were randomly assigned to observation group,control group one and control group two,26 cases each group.All the patients underwent the conventional closed thoracic drainage.The observation group took the intrapleural injection of Sapylin after lung recruitment maneuver;the control group one took the intrapleural injection of 50% high glucose after lung recruitment maneuver;the control group just took the closed thoracic drainage.The recurrent rate of spontaneous pneumothorax was analyzed. Results After 3 m,6 mand 12 mof treatment,the recurrence rate of spontaneous pneumothorax for three groups was not significantly different（P〉0.05）. Conclusion The recurrence rate of spontaneous pneumothorax based on pleurodesis with Sapylin,pleurodesis with 50% high glucose injection or closed thoracic drainage alone is not significantly different.
