密闭式与开放式吸痰在重型颅脑损伤患者中的疗效观察

Closed and open curative effect observation of sputum aspiration in patients with severe craniocerebral injury

目的探讨密闭式与开放式吸痰在重型颅脑损伤术后机械通气患者中的应用效果。方法选择2017年1~12月在我院神经外科住院治疗的重型颅脑损伤手术后行呼吸机辅助呼吸的患者60例为研究对象,随机分为两组,对照组30例使用常规开放式吸痰法,实验组30例采用密闭式吸痰,观察比较两组患者的临床效果。结果实验组患者的排痰量优于对照组,吸痰后SpO2显著高于对照组,吸痰后48h肺部感染率明显低于对照组,组间比较差异均有统计学意义(P<0.05)。结论密闭式吸痰对重型颅脑损伤术后机械通气患者能有效降低肺部感染率,对患者预后有积极作用,值得临床推广应用。

Objective To discuss the closed and open sputum suction in severe head injury postoperative effect of the treatment of patients with mechanical ventilation. Methods Between January 2017 and December 2017 in our hospital neurosurgery hospital treatment of severe head injury surgery after mechanical ventilation,60 patients with breathing machine accessory breathing,randomly divided into two groups,control group of 30 patients with conventional open sputum suction method,the experimental group 30 cases were treated by closed sputum suction,to compare the clinical effect of two groups of patients. Results The experimental group amount of expectoration drainage is better than that of control group,the experimental group after the sputum suction SpO2 observation group is significantly higher than than,the experimental group and control group 48 hafter sputum suction lung infection rates significantly lower than the control group,explain the experimental group is better than the control group,statistically significant difference（P〈0.05）. Conclusion Sputum suction conurbation sputum suction is superior to the open place lies in its greatly reduced postoperative pulmonary infection,for improving the prognosis of severe craniocerebral surgery have a positive effect,worthy of clinical popularization and application.
