HIF-1α在高龄COPD患者发病中的研究

Study of HIF-1α in Pathogenesis of COPD in Elderly Patients

目的探讨COPD高龄患者(年龄>80岁)血中HIF-1α的水平及其对COPD发病的影响。方法纳入年龄>80岁且符合COPD诊断标准的稳定期患者40例及AECOPD 40例,以及健康体检高龄老人30例。抽血查HIF-1α、动脉血PaCO2、PaO_2水平及肺功能。结果三组年龄、性别、体质量等指标无统计学差异。AEC0PD组、C0PD稳定期组、健康对照组三组的HIF-1α、动脉血PaCO2水平差异有统计学意义(P<0.01),其中AEC0PD组的HIF-1α、动脉血PaCO_2最高,稳定期组次之,健康对照组最低。三组间的动脉血PaO_2水平、FEV1占预计值百分比差异有统计学意义(P<0.01),其中AEC0PD组最低,稳定期组次之,健康对照组最高。结论 COPD稳定期及AECOPD的HIF-1α水平均明显升高,且与COPD肺功能损害密切相关。早期监测血HIF-1α水平有可能是观察COPD患者是否急性加重及治疗效果的的指标。

Objective To investigate the level of serum HIF-1α in elderly COPD patients（order than 80）and its effect on the pathogenesis of COPD. Methods 40 cases of COPD patients in stable phase and 40 cases of AECOPD who were all older than 80,as well as 30 healthy elders were included.Blood samples were collected for determination of HIF-1α,PaCO 2 and PaO 2 and pulmonary function was examined simultaneously. Results There was no statistically significant difference in age,sex and body mass among the three groups.There were statistically significant differences in HIF-1α and PaCO 2 among the three groups（ P 〈0.01）.HIF-1α and PaCO 2 of AECOPD group were the highest,followed sequentially by stable COPD group and control group.There were statistically significant differences in PaO 2 and FEV 1 %（FEV 1 predicted）among the three groups（ P 〈0.01）.Those of AECOPD group were the lowest,followed sequentially by stable COPD group and control group. Conclusion HIF-1α significantly increases in stable COPD as well as AECOPD,closely related to impaired pulmonary function.Early monitoring of serum HIF-1 α level helps to judge whether there is an acute exacerbation in COPD patients and assess therapeutic effect.
