莫西沙星治疗慢性阻塞性肺疾病急性加重期的疗效及对细菌清除率的影响

Efficacy of Moxifloxacin in the Treatment of Chronic Obstructive Pulmonary Disease and its Effect on Bacterial Clearance

目的探讨莫西沙星治疗慢性阻塞性肺疾病急性加重期(AECOPE)的疗效。方法选取2015年3月至2017年4月在我院治疗的AECOPD患者113例,根据治疗方案分为莫西沙星组(n=62)和左氧氟沙星组(n=51),观察两组治疗疗效、肺功能、超敏C反应蛋白(hs-CRP)、白细胞介素-8(IL-8)及肿瘤坏死因子!(TNF-!)水平及细菌清除情况。结果莫西沙星组治疗总有效率为79.03%,优于左氧氟沙星组,差异有统计学意义(P<0.05);莫西沙星组第一秒用力呼气容积(FEV1)和第一秒用力呼气容积占用力肺活量的比值(FEV1/FVC)分别为(2.40±0.46)L和(70.11±7.13)%,明显高于左氧氟沙星,差异有统计学意义(P<0.05);两组治疗后hs-CRP、IL-8及TNF-!均较治疗前有所降低,差异有统计学意义(P<0.05);莫西沙星组治疗后hs-CRP为(7.06±2.14)mg/L,明显低于左氧氟沙星组,差异有统计学意义(P<0.05);莫西沙星组细菌清除率为84.38%,明显高于左氧氟沙星组,差异有统计学意义(P<0.05)。两组患者均未出现严重不良反应。结论莫西沙星治疗AECOPD有较好的效果,可作为治疗AECOPD的首选药物。

Objective To investigate the efficacy of moxifloxacin in the treatment of acute exacerbation of chronic obstructive pulmonary disease（ AECOPD）.Methods 113 patients with AECOPD treated in our hospital from March 2015 to April2017 were selected and divided into two groups according to the treatment protocol： moxifloxacin group（ n = 62） and levofloxacin group（ n = 51）.The therapeutic effect,lung function,the levels of hs-CRPU,IL-8 and TNF-α,and the bacterial clearance condition were observed in the two groups.Results The total effective rate of moxifloxacin group was 79. 03%,which was better than that of levofloxacin group（ P〈0. 05）; In moxifloxacin group,the FEV1 and FEV1/FVCC ratio were（ 2. 40 ± 0. 46） L and（ 70. 11 ±7. 13） %,respectively,significantly higher than those in levofloxacin group（ P〈0. 05）; The bacterial clearance rate of moxifloxacin group was 84. 38%,significantly higher than that of levofloxacin group（ P〈0. 05）. There were no serious adverse reactions in both groups.Conclusion Moxifloxacin has a good effect on AECOPD and can be used as the first choice in the treatment of AECOPD.
