摘要
目的探讨脑钠肽(BNP)在不同窒息程度及窒息后心肌损害患儿中的表达。方法选取西安医学院第二附属医院产科2016-08~2017-12出生的窒息患儿40例,根据Apgar评分分为重度窒息组(10例)和轻度窒息组(30例),选择同期正常新生儿25例为对照组;再根据心肌损害标准将窒息患儿分为心肌损害组和无心肌损害组;所选对象均查脐动脉血气及静脉血BNP,肌钙蛋白I(cTnI),肌酸激酶同工酶(CK-MB),比较BNP在3组中的表达;分析BNP与血气中的pH值、BE值的相关性;分析BNP与cTnI、CK-MB的相关性。结果 1.重度窒息组中BNP高于轻度窒息组(P<0.01),轻度窒息组BNP值高于对照组(P<0.01);窒息组中BNP的表达与血气中的pH值、BE值成负相关;窒息组中BNP与cTnI、CK-MB成正相关。结论 BNP可间接反映窒息程度,可评估窒息后心肌损害程度。
Objective To explore the expression of brain natriuretic peptide(BNP) in children with different degree asphyxia and post-asphyxia myocardial damage.Methods Selected 40 children with asphyxiation who were born between August 2016 and December 2017 in the second affiliated hospital of Xi'an medical college.According to the Apgar score,they were divided into severe asphyxia group(10 cases) and mild asphyxia group(30 cases).Normal newborns were 25 controls.According to the criteria of myocardial damage,children with asphyxiation were divided into myocardial damage group and no myocardial damage group.The selected subjects examined umbilical arterial blood gas and venous blood BNP,troponin I(cTnI),and creatine kinase isoenzyme(CK-MB).The expression of BNP in the three groups was compared;the correlation between BNP and blood gas pH and BE values was analyzed;and the correlation between BNP and cTnI and CK-MB was analyzed.Results BNP in severe asphyxia group was higher than mild asphyxia group(P〈0.01);BNP in mild asphyxia group was higher than that in control group(P〈0.01);BNP expression in asphyxiation group was negative to pH value and BE value in blood gas Related;BNP in the asphyxia group positively correlated with cTnI and CK-MB.Conclusion BNP can indirectly reflect the degree of asphyxia;myocardial damage can be assessed after asphyxia.
