完全性右束支传导阻滞伴复合性ST-T改变特点探讨 被引量:2

摘要 目的探讨完全性右束支传导阻滞(CRBBB)伴复合性ST-T改变特点,提升对这种异常改变的认识。方法选择361例CRBBB患者,分为CRBBB伴复合性ST-T改变组(观察组)74例,CRBBB伴继发性ST-T改变组(对照组)287例,比较两组P-R间期、QRS波时限、Q-Tc间期、J-Tc间期、V1导联室壁激动时间(VAT)、V5导联S波宽度、ST-T改变特征以及心律失常发生情况。结果观察组95.95%(71/74)见于器质性心脏病患者,尤以冠心病、冠心病合并高血压病和(或)2型糖尿病多见,占69.01%(49/71);对照组39.37%(113/287)见于器质性心脏病(P<0.01)。观察组P-R间期、QRS波时限、Q-Tc间期、J-Tc间期、V1导联VAT均明显较对照组延长(P<0.01),各类早搏及一度房室阻滞等心律失常的发生率亦明显高于对照组(P<0.01)。复合性ST-T改变可以有多种表现形式,有单项T波低平、倒置、对称,ST段水平压低或延长等,也可以是ST段压低伴T波异常,个别合并急性心肌梗死可出现ST段抬高,但以ST段压低伴T波低平或倒置者多见(占39.19%)。结论 CRBBB伴复合性ST-T改变绝大多数是一种病理性改变,常为冠心病等器质性心脏病所致,多伴有J-Tc间期延长、早搏等,心律失常的发生率较高,应引起临床医师足够重视。
