弹性髓内钉内固定治疗儿童尺桡骨骨折

The treatment of children's fractures of ulna and radius with elastic intramedullary nails

目的探讨应用弹性髓内钉内固定治疗儿童尺桡骨骨折的临床疗效。方法应用弹性髓内钉治疗28例尺桡骨骨折患儿。术后3～6个月根据骨折愈合情况取出内固定物。结果 28例均获得随访,时间6～12个月。骨折均骨性愈合,时间8～15周。1例术后出现短期拇指背伸受限,1例患肢肘部出现钉尾激惹。术后均未发生再骨折、骨不连接、断钉及骨骺损伤等并发症。末次随访时根据肘关节HHS功能评分评定疗效:优26例,良2例。结论应用弹性髓内钉治疗儿童尺桡骨骨折具有创伤小、出血少、疼痛轻、愈合快、不损伤骨骺、并发症少等优点,是一种较为理想的治疗方法。

Objective To investigate the clinical effect of elastic intramedullary nailing for ulnar and radial fractures in children. Methods A total of 28 cases of children with ulnar and radial fractures were treated with elastic intramedullary nails. The implants were removed 3 ～ 6 months after operations when bony union. Results All the 28 patients were followed up for 6 ～ 12 months. All fractures were healed in 8 ～ 15 weeks. One patient presented with limited extension of the thumb after surgery,and 1 patient suffered from nail-tail irritation at the elbow. No complications such as refracture,bone nonunion,broken nails and epiphyseal injuries etc. At final follow-up,the elbow function was evaluated by the HHS score standard,26 cases were excellent,2 cases were good. Conclusions The treatment of elastic intramedullary nailing for ulnar and radial fractures in children is effective,because it has the advantages of less trauma,less bleeding,less pain,fast healing,no epiphysis injuries,less complications and so on.
