摘要
目的探讨多囊卵巢综合征(PCOS)患者血清维生素D结合蛋白(DBP)和25-羟基维生素D[25(OH)D]水平及其与患者代谢特征的关系。方法选取2016年2月至2017年2月在上海市普陀区妇婴保健院诊治的PCOS患者52例作为观察组,另外选取同期进行健康体检且月经规律的育龄期女性50人作为对照组,比较两组身高体重指数(BMI)、腰围/臀围比值、25(OH)D、DBP、瘦素、胰岛素抵抗指数(HOMA-IR)和胰岛β细胞功能指数(HOMA-β),并分析各因素与代谢指标的相关性。结果与对照组相比,观察组患者的BMI和腰围/臀围比值、25(OH)D、瘦素、HOMA-IR、HOMA-β、睾酮、黄体生成素、卵泡雌激素均显著升高,差异具有统计学意义(P<0.05),两组患者的DBP相比,差异无统计学意义(P>0.05);血清25(OH)D水平与HOMA-IR、HOMA-β、腰围/臀围比值、BMI和瘦素呈显著负相关(r=-0.928、-0.957、-0.982、-0.935、-0.926,P均<0.05)。结论 PCOS患者血清内25(OH)D水平与胰岛素抵抗及肥胖呈显著负相关。
Objective To investigate the relationship between the D binding protein(DBP),serum 25-hydroxy vitamin D[25(OH)D]and the metabolic characteristic in patients with polycystic ovary syndrome(PCOS)(P〈0.05).Methods 52 cases of PCOS were collected from February 2016 to February 2017 in Putuo Maternal and Child Health Care Hospital of Shanghai as observation group;50 cases health physical examination and regular menstruation period women were selected as control group at the same period.The BMI,waist circumference/hip ratio,levels of 25(OH)D,DBP,leptin,HOMAIR,HOMA-β,testosterone,luteinizing and follicle stimulating hormone of two groups were compared,and the correlation of various factors and metabolic index were analyzed.Results Compared with the control group,the BMI,waist circumference/hip ratio,levels of 25(OH)D,leptin,HOMA-IR and HOMA-β of the observation group were significantly increased(P〈0.05).The DBP levels of the two groups had no significant difference(P〈0.05).There was a significant negative correlation between the levels of 25(OH)D and HOMA-IR(r=-0.928,P=0.005),HOMA-β(r=-0.957,P=0.000),waist circumference/hip ratio(r=-0.982,P=0.002),BMI(r=-0.935,P=0.004) and leptin(r=-0.926,P=0.000).Conclusion The level of serum 25(OH)D,insulin resistance and obesity has significant negative correlation in patients with polycystic ovarian syndrome.
