摘要
为比较静脉铁剂与注输红细胞治疗严重产后贫血的效果,本研究选取于我院出现严重产后贫血的患者32例。随机分为铁剂治疗组(A)与红细胞悬液治疗组(B)。检查记录患者疲乏量表的自我评价、血液学检查及不良反应。研究表明,通过患者疲乏量表的自我评价显示干预后自基线至12周时间段内研究对象的体力疲乏评价无显著性改变(-0.64(95%CI:-3.25;2.11,p=0.63)),二者在改善患者自我感觉疲乏程度方面无显著性差异;在血液学检查中,干预开始时输注红细胞悬液能够暂时性地提高血红蛋白水平,但自第3周起始静脉给予铁剂组患者的血红蛋白水平显著增高,具有统计学意义(p〈0.05),网织红细胞计数与网红内血红蛋白量(CHr)的变化趋势基本一致,尤其是第1周内使用静脉铁剂研究对象的含量显著高于输注红细胞悬液的患者,具有统计学意义(p〈0.05)。本研究表明,对于治疗严重产后贫血,静脉铁剂优于输注红细胞悬液,值得在临床上推广使用。
In order to compare the effect of intravenous iron with infusion of erythrocytes,32 puerperas with postpartum anemia in our hospital were selected and randomly divided into group A who treated with intravenous iron and group B who treated with infusion of erythrocytes.We checked the patient's fatigue scale for self-evaluation,hematological examination,and adverse reactions.The results showed that the self-evaluation of the patient fatigue scale showed no significant changes in the physical fatigue of the subjects from baseline to 12 weeks after intervention(-0.64(95% CI:-3.25;2.11,p=0.63)),this also suggested that there was no significant difference between the two interventions in improving the degree of self-perceived fatigue in the patients.In the hematological test results,the infusion of erythrocyte suspension at the beginning of the intervention was able to temporarily increase the level of hemoglobin,but the hemoglobin level was significantly higher in the group of intravenous administration of iron from the first 3 weeks(P〈0.05),Reticulocyte count and net red hemoglobin(CHr)were basically the same,especially in the first week of the use of intravenous iron research object content was significantly higher than the infusion of red blood cell suspension in patients with statistically significant(P〈0.05).This study showed that,for the treatment of severe postpartum anemia,intravenous iron might better than injection of red blood cell suspension,it might worth to promote the use of clinical.
