TCRP术后应用屈螺酮炔雌醇预防子宫内膜息肉复发的临床效果观察

The effect observation of drospirenone and ethinylestradiol tablets in prevent of endometrial polyps recurrence after TCRP

目的观察宫腔镜下子宫内膜息肉电切术(TCRP)后应用屈螺酮炔雌醇片预防子宫内膜息肉复发的效果。方法选取医院收治的子宫内膜息肉患者90例,采用随机数字表法分为观察组和对照组,各45例。2组均行TCRP治疗,术后对照组给予一般对症治疗,观察组在对症治疗基础上给予屈螺酮炔雌醇片治疗;比较观察2组患者术前、术后3、6、12个月的月经量、子宫内膜厚度和术后复发率、不良反应发生率。结果术前2组月经量比较差异无统计学意义(P>0.05),术后3、6、12个月2组月经量均少于术前,且观察组少于对照组,差异均有统计学意义(P<0.05)。术前2组子宫内膜厚度比较差异无统计学意义(P>0.05)。2组术后3、6、12个月子宫内膜厚度均较术前减小,且观察组改善更加明显,差异有统计学意义(P<0.05)。观察组复发率明显低于对照组,差异有统计学意义(χ2=6.4800,P<0.05);2组不良反应发生率比较差异无统计学意义(P>0.05)。结论 TCRP术后应用屈螺酮炔雌醇片能够有效预防子宫内膜息肉复发,减少患者术后月经量,抑制子宫内膜增长,安全性相对较高,值得临床推广应用。

Objective To observe the clinical effect of drospirenone and ethinylestradiol tablets in prevent of endometrial polyps recurrence after TCRP. Methods Selected 90 cases patients with endometrial polyps,which was randomly divided into observation group and control group,each of 45 cases. 2 groups were treated by TRCP. After surgery,control group was treated by general symptomatic treatment. Observation group was treated by drospirenone and ethinylestradiol tablets on the base of general symptomatic treatment. After treatment,compared the menstrual volume,endometrial thickness,postoperative recurrence rate,incidence of adverse reactions of 2 groups before treatment and 3,6,12 months after surgery. Results The menstrual volume of 2 groups beofre surgery was no statistic significant（ P〉0. 05）. The menstrual volume of 2 groups 3,6,12 months after surgery was less than that before surgery,and the observation group was less than that of control group,the difference was statistic significant（ P〈0. 05）. The endometrial thickness of 2 groups before treatment was no statistic significant（ P〉0. 05）. The endometrial thickness of 2 groups 3,6,12 months after surgery was smaller than that before surgery,and the observation group was smaller than that of control group,the difference was statistic significant（ P〈0. 05）. The recurrence rate of observation group was lower than that of control group,the difference was statistic significant（ χ^2= 6. 4800,P〈0. 05）. The adverse reaction rate of 2 groups was no statistic significant（ P〉0. 05）. Conclusion Drospirenone and ethinylestradiol tablets can efficiency prevent the endometrial polyps recurrence after TCRP,reduce the postoperative menstrual volume,inhibition of endometrial growth,has a relatively high safety,is worthy of clinical application.
