滋肾调肝活血调周法联合二甲双胍治疗PCOS的疗效观察及对血清TNF-α、IL-6水平的影响

Clinical Observation on Curative Effect of Nourishing Kidney Regulating Liver and Activating Blood Circulation Adjusting Menstrual Cycle Method Combining with Metformin on PCODS and Influences on Serum TNF-α and IL-6 Levels

目的:观察滋肾调肝活血调周法联合二甲双胍治疗PCOS患者的临床疗效及对血清炎症因子IL-6、TNF-α水平的影响。方法:114例肾虚肝郁型多囊卵巢综合征患者随机分为中西药组(39例)、中药组(37例)、西药组(38例),中药组单纯运用滋肾调肝活血调周法、西药组单纯运用盐酸二甲双胍片、中西药组运用中药滋肾调肝活血调周法结合二甲双胍片治疗,各组治疗疗程均为3月。治疗结束后比较3组患者的临床疗效(月经恢复至正常的情况)以及治疗前后血清IL-6、TNF-α的变化情况。结果:治疗后,中西药组、中药组、西药组总有效率分别为94.87%、67.57%、47.37%,3组间总有效率比较,差异均有显著统计学意义(P<0.01)。治疗后3组血清IL-6、TNF-α水平均有不同程度降低,中西药组和中药组及西药组比较,降低更明显,差异有统计学意义(P<0.05);西药组IL-6水平与中药组比较,降低更明显,差异有统计学意义(P<0.05)。结论:滋肾调肝活血调周法联合二甲双胍可显著降低多囊卵巢综合征患者血清TNF-α、IL-6水平,改善临床症状。

Objective：To observe the curative effect and the changes of serum inflammatory factors IL-6 and TNF-α levels in patients with PCOS treated by Nourishing Kidney Regulating Liver and Activating Blood Circulation Adjusting Menstrual Cycle Method combining with metformin.Methods：114 PCOS patients with kidney deficiency and liver stagnation were randomly divided into observation group（ 37 cases）,Western Medicine group（ 38 cases）,Chinese and western medicine group（ 39 cases） by single blind method.All the three group were treated for three months.After the treatment,the curative effect of the three groups and the changes of serum IL-6 and TNF-α levels before and after treatment were compared.Results：After treatment,the total effective rate of western medicine group,Chinese medicine group and observation group was respectively94.9%,67.6% and 47.4%,The difference among the three groups was statistically significant（ P〈0.01）,the effect of observation group was better than the other two groups.The serum IL-6,TNF-α levels of the three groups were decreased in different degrees after treatment,the levels of observation group were lower than those of Chinese medicine group and Western medicine group（ P〈0.05）,IL-6 level of western medicine group was decreased significantly compared with the Chinese medicine group,the difference was statistically significant（ P〈0.05）.Conclusion：Nourishing Kidney Regulating Liver and Activating Blood Circulation Adjusting Menstrual Cycle Method combined with metformin can significantly reduce the serum IL-6 and TNF-αlevels in PCOS patients,improve clinical symptoms,so as to achieve the purpose of treatment.
