超声引导下关节腔药物注射治疗伴髋关节病变的强直性脊柱炎的临床疗效

Clinical effectiveness of ultrasound guided intra-articular injection on hip joint lesion in patients with ankylosing spondylitis

目的观察超声引导下关节腔药物注射治疗伴髋关节病变的强直性脊柱炎的临床疗效。方法采用前瞻性设计方案选取2014年1月~2016年12月我院门诊治疗且符合纳入标准的48例患者,分别从临床疗效、视觉模拟评分(VAS)和髋关节Harris评分及安全性评估等三个方面评定患者的临床疗效。结果本研究所有患者均获得随访,随访8~43个月,平均(27.67±10.13)个月;与治疗前比较,治疗后3个月患者髋关节ROM、VAS评分和髋关节Harris评分均有明显改善,差异有统计学意义(P<0.05);治疗后3个月ESR及CRP等理化指标也有不同程度的变化,差异有统计学意义(P<0.05)。与治疗前比较,治疗后3个月患者肝功及肾功相关指标无明显变化,差异无统计学意义(P>0.05)。结论超声引导下关节腔药物注射治疗可以直接且有效清除关节腔内的致病因素,缓解疼痛、减缓病程发展,是治疗伴髋关节病变的强直性脊柱炎的安全而有效的措施。

Objective To investigate the clinical effectiveness of ultrasound guided intra-articular injection on hip joint lesion in patients with ankylosing spondylitis. Methods Under the prospective design, 48 patients in the department of out-patients in our hospital who met the inclusive criteria were selected. The assessment of clinical effectiveness in- cluded clinical effectiveness, visual analogue scale （VAS） and hip joint Harris scale and safety assessment. Results All the patients in this study were followed up. The follow-up period varied from 8 months to 43 months, averaged （27.67± 10.13） months. Compared with those before treatment, hip joint ROM, score of VAS and score of hip joint Harris scale improved significantly 3 months after treatment（P〈0.05）. ESR and CRP changed differently and significantly 3 months after treatment（P〈0.05）. Compared with those before treatment, indicators of liver function and renal function did not change significantly 3 months after treatment（P〉0.05）. Conclusion Ultrasound guided intra-articular injection could clean the pathogenic factors directly and effectively. It could relieve the pain and slow down the disease development. It would be a safe and effective method for hip joint lesion in patients with ankylosing spondylitis.
