The Role of MRI Diffusion Weighted Imaging in Differential Diagnosis of Primary Central Nervous System Lymphoma and High Grade Glioma

摘要 目的:探讨DWI在原发性神经系统淋巴瘤(PCNSL)与高级别胶质瘤(HGG)鉴别诊断中的价值。材料与方法:回顾性分析13例PCNSL和25例HGG患者的常规MRI表现、DWI表现,并测量肿瘤实质的ADC值,进行统计学分析。结果:13例PCNSL在DWI上肿瘤实质10例表现为均匀高信号,2例呈稍高信号,1例呈等信号,肿瘤实质平均ADC值为(0.75±0.04)×10-3mm^2/s。25例HGG在DWI上肿瘤实质17例呈高信号,5例呈稍高信号,3例呈等、稍低信号,肿瘤实质平均ADC值为(1.08±0.63)×10^(-3)mm^2/s。PCNSL与HGG肿瘤实质ADC值差异有统计学意义(t=2.13,P<0.05)。结论:测量肿瘤实质ADC值并结合常规MRI有助于PCNSL与HGG的鉴别诊断。 Objective: To explore DWI in primary central nervous system lymphoma(PCNSL) and high grade gliomas( HGG) in the differential diagnosis value. Materials and Methods: A retrospective analysis of 13 cases of PCNSL and 25 cases of HGG patients with normal MRI performance,DWI performance,and the measurement of ADC value of tumor parenchyma were performed,and statistical analysis was performed. Results: The 13 cases of PCNSL tumor parenchyma in DWI 10 cases showed a uniform high signal on DWI,2 cases showed a slightly higher signal,1 case presented,and the average ADC value of the tumor was(0. 75 + 0. 04) ×10^-3mm^2/s. The tumor parenchyma of 25 cases of HGG in DWI 17 cases showed high signal on DWI,5 cases were slightly higher signal,3 cases were presented with equal and lower signal,and the average ADC value of tumor was(1. 08 + 0. 63) ×10^-3mm^2/s. Conclusion: The differential diagnosis of PCNSL and HGG was helped by measuring the ADC value of tumor parenchyma and combining with conventional MRI.
