血清炎症指标对老年急性心力衰竭并发心脏不良事件患者的预测价值分析

Predictive value analysis of serum inflammatory markers in elderly patients with acute heart failure complicated by adverse cardiac events

目的分析血清炎症指标对老年急性心力衰竭(AHF)并发心脏不良事件的预测价值。方法 98例老年AHF患者,根据在院期间是否发生心脏不良事件分为研究组(发生心脏不良事件,40例)与对照组(未发生心脏不良事件,58例)。检测并比较两组患者肿瘤坏死因子-α(TNF-α)、N末端B型利钠肽原(NT-Pro BNP)、白细胞介素-6(IL-6)水平,并分析老年AHF并发心脏不良事件的危险因素。结果研究组NT-Pro BNP为(5.52±0.52)μg/L、TNF-α为(21.30±5.20)ng/L、IL-6为(19.55±4.65)ng/L,均高于对照组的(3.11±0.30)μg/L、(12.75±4.52)ng/L、(13.28±3.20)ng/L,差异具有统计学意义(P<0.05)。经Logistic分析,NT-Pro BNP≥4.5μg/L、TNF-α≥18.0 ng/L、IL-6≥16.0 ng/L是老年AHF并发心脏不良事件的独立危险因素(P<0.05)。结论血清炎症指标对老年AHF并发心脏不良事件具有显著的预测价值。

Objective To analyze the predictive value of serum inflammatory markers in elderly patients with acute heart failure（AHF） complicated by adverse cardiac events. Methods A total of 98 elderly patients with AHF were divided into the research group（occurred adverse cardiac events） for 40 cases and the control group（no adverse cardiac events） for 58 cases. The tumor necrosis factor-α（TNF-α）, N terminal B type natriuretic peptide（NT-Pro BNP） and interleukin-6（IL-6） level between the two groups were detected and compared, and the risk factors of elderly AHF complicated by adverse cardiac events were analyzed. Results The levels of NTPro BNP, TNF-α and IL-6 in the research group were（5.52±0.52） μg/L,（21.30±5.20） ng/L and（19.55± 4.65） ng/L, which were significantly higher than（3.11±0.30） μg/L,（12.75±4.52） ng/L and（13.28±3.20） ng/L in the control group, the differences were statistically significant（P〈0.05）. After Logistic analysis, NT-proBNP ≥4.5 ng/L, TNF-α≥18.0 ng/L and IL-6≥16.0 ng/L were independent risk factors of elderly patients with AHF complicated by adverse cardiac events（P〈0.05）. Conclusion Serum inflammatory markers have significant predictive value for elderly patients with AHF complicated by adverse cardiac events.
