摘要
目的观察高血压病患者和健康对照者空腹血清游离脂肪酸(FFA)和动脉弹性的差异,以及高血压患者应用阿托伐他汀治疗后FFA和动脉弹性的变化。方法 218例患者,按照基础疾病分为高血压组(103例)和非高血压组(115例),分析比较两组患者的一般资料,比较两组患者的FFA和上臂-脚踝脉搏波传导速度(baPWV)水平。高血压组患者应用阿托伐他汀治疗6个月,比较治疗前后的收缩压(SBP)、舒张压(DBP)、FFA及baPWV水平。结果高血压组患者的高血压家族史占比、体质量指数(BMI)、心率、腰围、SBP、DBP、总胆固醇(TC)、甘油三酯(TG)以及低密度脂蛋白胆固醇(LDL-C)水平均显著高于非高血压组,差异具有统计学意义(P<0.05)。高血压组患者FFA(0.56±0.19)mmol/L和baPWV(1589±214)cm/s显著高于非高血压组的(0.33±0.18)mmol/L、(1386±207)cm/s,差异具有统计学意义(P<0.05)。治疗后,高血压组患者的SBP、DBP及baPWV水平均低于治疗前,差异具有统计学意义(P<0.05)。结论原发性高血压患者空腹血清FFA升高,动脉弹性降低,之间存在相关性。阿托伐他汀的干预治疗可降低FFA,一定程度改善高血压患者的动脉弹性。
Objective To observe the difference of free fatty acid(FFA) and arterial elasticity in the fasting serum of hypertensive patients and healthy controls, as well as the changes of FFA and arterial elasticity after the treatment of Atorvastatin in hypertensive patients. Methods A total of 218 patients were divided into hypertension group for 103 cases and non hypertension group for 115 cases according to the underlying disease. The general data of the two groups were analyzed and compared. The FFA and upper arm ankle pulse wave velocity(baPWV) levels of the two groups were compared. The patients in the hypertension group were treated with Atorvastatin for 6 months. The systolic blood pressure(SBP), diastolic blood pressure(DBP), FFA and baPWV levels were compared before and after treatment. Results The proportion of hypertension family history, body mass index(BMI), heart rate, waist circumference, SBP, DBP, total cholesterol(TC), triglyceride(TG), and low density lipoprotein cholesterol(LDL-C) levels in hypertensive patients were significantly higher than those in non hypertension group, the FFA and baPWV in the hypertensive group were(0.56±0.19) mmol/L and(1589± 214) cm/s were significantly higher than(0.33±0.18) mmol/L and(1386±207) cm/s in the non hypertensive group, the differences were statistically significant(P〈0.05). After treatment, the levels of SBP, DBP and baPWV in patients with hypertension were significantly lower than those before treatment, the differences were statistically significant(P〈0.05). Conclusion In patients with essential hypertension, fasting serum FFA increased, arterial elasticity decreased, and there was a correlation between them. Atorvastatin intervention can reduce FFA and improve arterial elasticity of hypertensive patients to some extent.
