老年慢性阻塞性肺疾病患者应用综合康复护理措施对肺功能的影响

Effect of comprehensive rehabilitation nursing measures on pulmonary function in elderly patients with chronic obstructive pulmonary disease

目的探讨综合康复护理措施对老年慢性阻塞性肺疾病(COPD)患者康复与肺功能的效果。方法选取100例COPD老年患者随机分为2组,对照组给予常规护理,实验组则加施综合康复护理,比较2组患者血气学指标、肺部功能、睡眠质量、生存质量与护理满意度。结果实验组患者干预后p(CO_2)水平、主观睡眠质量、睡眠潜伏期、睡眠持续性、习惯性效率、使用睡眠药物评分、匹兹堡睡眠质量指数量表(PSQI)总分、日常生活能力、社会活动状况、抑郁心理症状、焦虑心理症状评分与生存质量总分显著低于对照组,而p(O2)、第1秒用力呼气容积(FEV_1)、肺活量(FVC)、FEV_1/FVC、呼气峰流量(PEF)、呼气中断流速(MMEF)与6 min步行距离(6MWT)水平显著高于对照组,护理满意度显著优于对照组,差异均有统计学意义(P<0.05)。结论综合康复护理措施可显著提升老年COPD患者康复效果与肺功能改善效果,并提升护理满意度,值得在临床推广应用。

Objective To explore effect of comprehensive rehabilitation nursing measures on rehabilitation and pulmonary function in elderly patients with chronic obstructive pulmonary disease（ COPD）. Methods A total of 100 elderly patients with COPD were randomly divided into two groups. The patients in the control group were given routine nursing,while the patients in the experimental group were treated with comprehensive rehabilitation. Blood gas parameters,lung function,sleep quality,quality of life and nursing satisfaction of two groups were compared. Results After treatment,the levels of p（ CO2）,subjective sleep quality,sleep latency,sleep persistence,habitual efficiency,use of sleep drug score,PSQI score,daily living ability,social activity status,depression psychological symptoms,anxiety and psychological symptoms score and life quality scores were lower,while p（ O2）,forced expiratory volume in one second（ FEV1）,forced vital capacity（ FVC）,FEV1/FVC,peak expiratory flow（ PEF）,maximum midexpiratory flow（ MMEF） and 6 minute walk test（ 6 MWT） were significantly higher than that of the control group（ P〈0. 05）. The nursing satisfaction was significantly higher than that of the control group（ P〈0. 01）. Conclusion Comprehensive rehabilitation nursing measures can improve rehabilitation efficacy and pulmonary function of elderly patients with COPD,which can improve satisfaction,so it is worthy of clinical application.
