糖耐量异常人群血清hs-CRP、Hcy、CysC水平与颈动脉粥样硬化的相关性研究 被引量:7

摘要 目的联合检测了三种不同糖耐量人群血清超敏C反应蛋白(hsCRP)、同型半胱氨酸(Hcy)、胱抑素C(CysC)水平,探讨早期动脉粥样硬化的相关危险因素,寻找预防、评估和治疗动脉粥样硬化的可靠实验室指标。方法选取40例糖耐量正常(NGT)者为NGT组、50例糖耐量异常(IGT)患者为IGT组、50例新诊断2型糖尿病(T2DM)患者为T2DM组,分别检测了三组人群血清hs-CRP、Hcy、CysC水平及生化指标,用彩色多普勒超声仪测定颈动脉内膜中层厚度(IMT)。结果三组人群血清hs-CRP、Hcy、CysC水平及颈动脉内膜IMT中层厚度逐渐增高,IGT组、T2DM组的Hcy、hs-CRP、CysC水平均显著高于NGT组,且T2DM组与IGT组比较差异亦有统计学意义(P<0.05),组间两两比较差异有统计学意义(P<0.05)。多元Logistic回归分析显示,血清低密度脂蛋白(LDL-C)、hs-CRP、Hcy、CysC、胰岛素抵抗指数(HOMA-IR)是颈动脉粥样硬化的独立危险因素。结论血清hs-CRP、Hcy、CysC水平在糖代谢异常早期就已经升高,同时并参与了糖尿病动脉硬化的进程。 Objective To detect the a combination of three different glucose tolerance crowd hypersensitive serum c-reactive protein(hsCRP),homocysteine(Hcy),cystatin C(CysC)level,to explore the related risk factors of early athero sclerosis,looking for prevention,assessment and treatment of atherosclerosis and reliable laboratory indexes.Methods To choose 40 cases with normal glucose tolerance(NGT)for NGT group,50 cases of patients with abnormal glucose tolerance for IGT group,50 cases of patients with newly diagnosed type2 diabetes mellitus(T2 DM)group,three groups respectively to detect the serum hs-CRP,Hcy,CysC level and biochemical indicators,using color doppler ultrasound instrument determination of carotid intima-media thickness(IMT).Results Three groups of serum hs-CRP,Hcy,CysC level and IMT of carotid artery lining thickness gradually increased,IGT group,the T2 DM group of Hcy,hs-CRP,CysC level were significantly higher than that of NGT group,and T2 DM with IGT group comparative differences are statistically significant(P〈0.05)between two groups to compare difference was statistically significant(P〈0.05).Multiple Logistic regression analysis showed that serum LDL-C,hs-CRP,Hcy,CysC,insulin resistance index(Homa-IR)and independent risk factors for carotid atherosclerosis.Conclusion Serum hs-CRP,Hcy,CysC levels have been elevated in the early stage of glucose metabolism,and have been involved in the process of diabetic atherosclerosis.
