老年骨质疏松性骨折患者血清BGP、BALP、TRACP-5b表达的价值研究

The value of serum osteocalcin,BALP and TRACP-5b expression in elderly patients with osteoporotic fracture

目的探讨老年骨质疏松性骨折患者血清骨钙素、骨特异性碱性磷酸酶(BALP)、抗酒石酸盐酸性磷酸酶异构体5b(TRACP-5b)水平及其临床意义。方法选择该院收治的116例老年骨质疏松性骨折患者作为骨折组,再选择同期收治的110例老年骨质疏松未发生骨折患者作为对照组,检测两组患者血清骨钙素、BALP以及TRACP-5b水平,并分析骨密度水平与骨钙素、BALP、TRACP-5b水平相关性,采用ROC曲线分析血清骨钙素、BALP以及TRACP-5b水平对骨质疏松骨折患者的预测价值。结果骨折组患者腰椎正位、股骨颈以及股骨粗隆BMD水平均显著低于对照组(P<0.05);骨折组患者血清骨钙素、BALP以及TRACP-5b水平均显著高于对照组(P<0.05);骨质疏松骨折患者BMD值与骨钙素、BALP、TRACP-5b呈显著负相关(P<0.05);ROC曲线分析结果显示骨钙素、BALP、TRACP-5b预测老年骨质疏松患者骨折发生曲线下面积分别为0.729、0.887、0.948。结论老年骨质疏松患者血清骨钙素、BALP、TRACP-5b与骨密度水平呈明显负相关关系,且血清骨钙素、BALP、TRACP-5b能够有效预测骨质疏松患者骨折发生风险,对于防止老年骨质疏松骨折的预防具有着重要价值。

Objective To investigate serum osteocalcin,bone specific alkaline phosphatase（BALP）,tartrate resistant acid phosphatase 5 b（TRACP-5 b）level and its clinical significance in elder osteoporotic fracture patients.Methods In our hospital 116 cases of elderly patients with osteoporotic fractures were selected as the fracture group,and 110 cases of senile osteoporosis without fracture patients as control group,for the two groups of patients the serum levels of BGP,BALP and TRACP-5 blevels were detected,and to analyze the correlation between bone mineral density and the levels of BGP,BALP,TRACP-5 b,analysis serum BGP,BALP and TRACP-5 blevels in patients with fracture of the predictive value of osteoporosis by ROC curve.ResultsThe positive position of the lumbar spine,the neck of femur and the level of BMD in the femoral trochanter were significantly lower in the fracture group than in the control group（P〈0.05）.The levels of serum BGP,BALP and TRACP-5 bin the fracture group were significantly higher than those in the control group（P〈0.05）.The BMD value of osteoporotic fracture patients was negatively correlated with BGP,BALP and TRACP-5 b（P〈0.05）.ROC curve analysis showed that the area under the fracture curve of BGP,BALP and TRACP-5 bin elderly osteoporosis patients was 0.729,0.887 and 0.948.Conclusion The serum of patients with osteoporosis in elderly patients with BGP,BALP,TRACP-5 band bone density were significant negative correlation,and the serum BGP,BALP and TRACP-5 bcan effectively predict osteoporosis fracture risk,for the prevention of osteoporotic fracture has important value.
