中国西南地区社区人群血清尿酸水平与糖尿病发病的相关性及其性别差异性分析 被引量:6

摘要 目的探讨中国西南地区社区人群血清尿酸(UA)水平与糖尿病发病的相关性及性别的差异。方法采用整群随机抽样的方法,选取四川泸州地区行健康体检的3 830例社区居民进行横断面研究,采用尿酸酶比色法测定血UA水平,将所有受试者根据血UA水平分为低UA(男性≤148μmol/L,女性≤88μmol/L)、正常UA(男性149~416μmol/L,女性89~357μmol/L)、高UA(男性≥417μmol/L,女性≥358μmol/L)三个水平。分析不同血UA水平者的糖尿病发病情况,分析男性、女性不同UA水平者糖尿病发病情况,采用Logistic回归法分析血UA水平对糖尿病发病的影响。结果 3 830例受试者的血UA水平为(323.5±16.9)μmol/L。低UA水平者187例、正常UA水平者2 629例、高UA水平者1 014例。高UA水平者糖尿病发病比例高于正常UA和低UA水平者(P均<0.01)。男性中低UA水平者糖尿病发病比例高于正常UA及高UA水平者(P均<0.01);女性中高UA水平者糖尿病发病比例高于正常UA及低UA水平者(P均<0.01)。Logistic回归分析显示,与正常UA水平者相比,高UA水平者糖尿病的发病风险OR为1.5(P<0.01)。结论中国西南地区社区人群血UA水平增高时糖尿病发病较高,男性低UA水平、女性高UA水平与糖尿病发病有关。 Objective To investigate the correlation between serum uric acid( UA) levels and diabetes mellitus( DM) incidence and gender differences in community population of southwestern China. Methods Using a cluster random sampling method to select 3 830 community residents in Luzhou area of Sichuan for cross-sectional study. The urinary enzyme colorimetric method was used to determine the blood UA level. According to level of the blood UA,all subjects were divided into the low UA group( male ≤ 148 μmol/L,female ≤ 88 μmol/L),normal UA group( male 149-416μmol/L,female 89-357 μmol/L),and high UA group( male ≥ 417 μmol/L,female ≥ 358 μmol/L). We analyzed the incidence of diabetes in different blood UA levels,as well as the incidence of diabetes in males and females with different UA levels. Using the Logistic regression method to investigate the effect of blood UA levels on the incidence of diabetes.Results The blood UA level of 3 830 subjects was( 323. 5 ± 16. 9) μmol/L. There were 187 patients with low UA level,2629 patients with normal UA level,and 1014 patients with high UA level. The incidence of diabetes in patients with high UA level was higher than that in patients with normal and low UA( P〈0. 01). The incidence of diabetes in men with low UA level was higher than that of men with normal and high UA( P〈0. 01). The incidence of diabetes in women with high UA level was higher than that of women with normal and low UA( P〈0. 01). Logistic regression analysis showed that the risk of DM was 1. 5 in patients with high UA levels as compared with those with normal UA( P〈0. 01). Conclusion The incidence of diabetes is higher when the blood UA level increases in Chinese community,and the low UA level in men and the high UA level in women are associated with the onset of diabetes.
